One of my favorite ways to explain lipedema to patients - it's like a tattoo!

Tattoos may fade over time, but they don't go away. Similarly, lipedema fat doesn't really go away over time despite attempts at diet and exercise.

We use special lasers to break down tattoo pigment so your body can deal with them. With lipedema liposuction, we are physically extracting the fat because your body can't break it down.

I hope this video is a helpful way to think about lipedema fat because many patients can get frustrated with their lack of progress during diet and exercise and ultimately it's not your fault!

Should I get my veins treated before lipedema surgery? We often get this question for our lipedema surgeon and the answer might surprise you.

Spider veins or varicose vein treatment are performed by a vascular surgeon and the two most common procedures are vein ablation and vein stripping. These procedures can target dilated or large veins that contribute to swollen legs and achiness in legs from edema. Patient want to know if this is somethin that they should have done before their lipedema liposuction treatment.

For both procedures, compression garments are needed after the procedures to help with calf swelling. Both procedures are typically performed as awake surgery with numbing medicine.

During awake liposuction procedures, scar tissue from vein treatment can cause scar tissue that makes it more difficult to get numb during surgery. For this reason, we prefer to delay venous insufficiency treatment until after the liposuction for lipedema is performed. The one case that we prefer to treat the veins first is when large varicose veins are present and these can present a bleeding risk during liposuction. In this case, we prefer to have the veins treated initially to make lipedema surgery safer for our patients.

Lipedema is a chronic disease and treatment understandably provides value to the patient who gets to experience a decrease in their lipedema pain and swollen legs after the surgery. The lipedema surgeon benefits financially from either the patient who does self pay for lipedema liposuction, or through insurance covering lipedema surgery. What I think is often overlooked are the family members, the patient's employer, and the insurance company that also gain value from lipedema reduction surgery.

How many patients feel pain during awake lipo?...it's probably less than you think. Everyone is a little bit different, but most patients feel small pokes, maybe "pulling and tugging" but very few describe feeling like awake lipo hurts.

We use a combination of oral medication to relax you before the surgery as well as numbing medication that numbs the fat during the procedure so there's less pain in the fat during lipo. Most procedures last about 2-3 hours and are pretty straight forward for 9 out of 10 patients resulting in awake lipo being very comfortable.
