Dear Valued Patient:
Today, we are announcing the most difficult decision in the history of this organization: the planned closure of Hoskinson Health & Wellness Clinic.
From the very beginning, this organization was built around a belief that the people of Wyoming deserved access to exceptional healthcare close to home. We set out to create something very ambitious: a place where patients in our rural community could access advanced care, specialty providers, prevention programs, and modern medical technology without having to leave the region.
Over the past several years, we invested heavily in that mission. Our exceptional staff has elevated the standard of local healthcare. We recruited highly skilled providers, expanded our services rapidly, and worked to build a model we believed could meaningfully improve healthcare access in Wyoming.
Unfortunately, despite those efforts and investments, we have reached the difficult conclusion that the organization is no longer financially sustainable. This decision did not come lightly and came with exhaustive deliberation. Our leadership is focused on executing an orderly, compassionate and deeply responsible transition for our patients, staff, and the community we so deeply appreciate.
Hoskinson Health & Wellness Clinic will cease seeing patients as of July 31, 2026.
Your Care After July 31
Our last day of patient appointments is July 31, 2026. Between now and then, our full team remains committed to your care. We strongly encourage you to establish care with a new provider before our closing date so there is no interruption to your healthcare. We are happy to assist with referrals and will transfer your records directly to your new provider upon your request. We understand that finding a new provider can be challenging, and we do not take lightly the disruption this may cause. If you are having difficulty locating a local provider or need assistance with a specialist referral, please contact our clinic before July 31, 2026, and we will do our best to help you through this transition.
Your Prescriptions
If you are on ongoing medications, please contact us as soon as possible. Do not wait until July. We want to make sure every patient on medication has a plan before we close. Our pharmacy can be reached at (307) 387-9850.
Your Medical Records
Your records belong to you, and you will always have access to them. Before August 31, 2026, contact our office directly at: (307) 387-9850. To ensure continuity of your care, we strongly encourage you to request a copy of your records, or to have them transferred to another healthcare provider, on or before July 17, 2026 to allow adequate processing time before the Clinic closes.
If you do not request your records before the Clinic closes, your records will be securely maintained by our contracted medical records custodian:

Your records will be preserved for a minimum of ten (10) years in accordance with Wyoming law. There is no deadline for requesting them. They will be there whenever you need them.
