Late one holiday evening, our hospice received an urgent referral for a patient whose only wish was to spend his final moments at home. Time was critical. The patient required durable medical equipment and comfort medications to safely discharge.

The challenge was significant. The patient required durable medical equipment and comfort medications to safely discharge, and time was critically limited. Multiple hospice agencies had already been contacted, but none were able or willing to accept the patient for a same-day admission on a holiday evening.

Our team did not hesitate.

Within minutes of receiving the referral, staff across the organization mobilized. Coordination began immediately to secure the necessary equipment, obtain medications, and ensure a safe transition home. Despite the late hour and the holiday, team members worked tirelessly behind the scenes to remove every barrier that stood between this patient and his wish to go home —making calls, arranging deliveries, coordinating logistics, and preparing for admission.

A hospice nurse dedicated several hours that evening to the patient and family. She ensured that all equipment and medications were in place, provided hands-on care, educated the family, and made certain the patient was comfortable and supported.
Because of the extraordinary efforts of the entire team, the patient was able to return home that night.

Surrounded by the comfort of his home and loved ones, his final wish was honored. He passed peacefully where he most wanted to be.
Moments like this reflect the commitment of our hospice team. When time is short and circumstances are challenging, we work together to ensure patients and families receive the compassionate care and support they deserve.
