Dear Dr. Escobar,
Thank you for providing dental hygine kits that we include in the backpacks we provide to needy students in south Santa Clara County. As you know, children in south Santa Clara County suffer disproportionally from poor oral heath, and any preventive measures (like dental hygine kits0 are always appreciated by parents.

The Ladies of Charity of Morgan Hill is proud to provides backpacks filled with school supplies to at-risk middle and high school students. Very few groups focus on these students as their backpacks need to be sturdier and require school supplies that differ from elementary students - these backpacks are significantly more expensive to fill.

This year we filled 250 backpacks which were distributed as follows:
Latino Family Fund of Gilroy Youth Program-50
Community Agency for Resources, Advocacy and Services-50
Gilroy and Morgan Hill Special Needs students-25
Ca-Soap of Gilroy and Morgan Hill Unified School District - 125

Periodontal diseases are infections of the gums, which gradually destroy the support of your natural teeth. There are numerous disease entities requiring different treatment approaches. Dental plaque is the primary cause of gum disease in genetically susceptible individuals. Daily brushing and flossing will prevent most periodontal conditions.

Discover a New Level of Care at Jernell Esocbar, DDS for Your First Visit

Your initial appointment usually will consist of an examination and review of your treatment options. Usually, treatment can be done or started the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day. Please assist us by providing the following information at the time of your consultation:

💠 Any x-rays taken by a previous dentist, if applicable.
💠 A list of medications you are currently taking.
💠 If you have dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
💠To make things easier and save time when you arrive on your first visit please print and fill out our patient intake form, which you can download here.

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian. Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).

X-Rays
If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional x-rays are necessary, they can be taken at our facility.
