Premier Pediatrics, LLC - Glen Burnie

Premier Pediatrics, LLC - Glen Burnie Shannon L. West, M.D. Comprehensive care for your children - from birth to age 21.

07/18/2022

It is with much regret that we inform you that Premier Pediatrics will be closing permanently on August 24th, 2022. We will honor all appointments until then and do have the capacity to accommodate some new appointment until that date. Appointments scheduled AFTER August 24th should be scheduled with a new provider ASAP to prevent delays in well child care or immunizations.

Dr. West will be available at The Child and Teen Wellness Center in Owings Mills https://www.ctwcmd.com some time in September. Although this location will not be convenient for most current patients, you may consider an appointment there if unable to obtain an appointment with a new provider in a timely fashion so that continuity of care can be maintained. Most, but not all insurances will be accepted at that location.

To obtain immunization records in the near future please do so through the patient portal https://mycw50.eclinicalweb.com/portal5723/jsp/100mp/login_otp.jsp or https://app.myirmobile.com/auth/register?state=MD

To obtain a copy of the complete medical record please download a Release of Medical Records from our websitehttps://www.prempedsllc.com/_files/ugd/c5e007_b17ece0d765942b197fb51706f8ad48d.pdf , complete one for each child, and fax to (936) 236-6424. The form can also be mailed to:
Premier Pediatrics Records Request
PO Box 1666
Owings Mills, MD 21117

Records will be prepared as expediently as possible but may take up to 30 days to prepare.

Update: Please send individual questions to busregistration@aacps.org

REMINDER: The registration link sent to families to for bus transportation for the upcoming year expires after Friday, May 13. Messages are only being sent to the primary contact on a student’s account. Families who have not yet completed registration for their student should click on the link at the very bottom of the message and fill this form out as soon as possible, regardless of whether their student needs transportation or not. Families were sent a separate form for each child. Forms are student-specific and cannot be forwarded to others.

Students whose families do not complete registration forms will not be eligible to ride a school bus for the entire 2022-23 school year even if they reside in the transportation zone. Families who believe their student will need bus transportation at any point in the upcoming school year are encouraged to indicate on the form that they wish to have bus service.

Please note that prekindergarten and kindergarten students, students who have IEPs that contain transportation accommodations, and students designated as homeless will automatically receive transportation. No registration for those students is needed.

Families do not need to complete forms for students who take shuttles from their schools or homes directly to the Centers of Applied Technology, but must complete the bus registration form for student transportation to the home school.

Anyone needing support with completing the form should contact their student’s school.

Watch for the email!

Anne Arundel County Public Schools is implementing a mandatory bus registration process for the upcoming school year. Beginning on May 4, families whose students are eligible for bus transportation, based on the student’s primary address, will receive an encrypted email from AACPS containing a secure link to the bus registration form for each student who is eligible for bus transportation. Families should click on the link in the message and fill this form out as soon as possible, regardless of whether their student needs transportation or not. Families will receive a separate form for each student. Forms are student-specific and cannot be forwarded to others.

Students whose families do not complete registration forms will not be eligible to ride a school bus for the entire 2022-23 school year even if they reside in the transportation zone.

Please note that prekindergarten and kindergarten students, students who have IEPs that contain transportation accommodations, and students designated as homeless will automatically receive transportation. Students who take shuttles from their schools or homes directly to the Centers of Applied Technology do not need to complete a form for this purpose, but must complete the bus registration form for transportation to the home school.

Families of students at the county's four charter or contract schools will receive transportation information from their student's school later in the year.
