The aging effect on the lower eyelid involve multiple different aspect of the lower eyelid anatomy.
Fat around the eyes is held back by a barrier called the orbital septum. This barrier can be weak genetically or weaken over time which causes the fat to pseudoherniate and create a bulge under the lower eyelids. There are three separate fat pockets in the lower lid.
The volume in the area of the lower lids and midface gradually decreases over time leaving a depression known as the tear troughs, which is the exposed orbital rim bone below the orbital fat packets but above the cheek fat or malar fat pad. This creates a valley of depression which needs to have volume replaced. This can be done with repositioning of the lower eyelid fat or fat injection.
The third component of lower lid aging is the skin which ages in this area like it does everywhere, however the skin in this area is particularly thin and can become excessively lax and creepy. Sun damage, smoking and genetics all play a role in how quickly the sighs of aging appear. The skin can be tightened with chemical peels, laser or skin removal.
Addressing all three of these areas leads to best results for a smooth lid-cheek junction and natural youthful appearance.
