PLEASE READ CAREFULLY!! We've done our best to make this as clear & as simple as possible.

Below is the link to begin completing the athletic online forms for the 2024-25 school year.

This year, we have made it possible for you to upload your student's physical form instead of turning it in manually. The pictures explain how to complete this step.

All other forms will be completed electronically like we have done in the past.

Please know that your student will not become "eligible" or "cleared" on the forms until Coach Arendt or myself approves the forms. When you have completed the forms AND we have approved them, your students name will turn GREEN instead of staying RED. If you log in and your students name is RED, you have not completed all the forms or we have not approved them yet.

On the medical history electronic form, please click "NO" on the ECG question toward the end. However, if your student has a heart abnormality and you would like to get an ECG for clarification, then you would click "YES" and bring us the results of the ECG to be cleared.
