I'm in the confidence business.

My patient is two months out from rhinoplasty and fat transfer to the chin, still the very early days of swelling. But the glow of confidence is undeniable. When I asked her if I could share her before & afters she said "Dr. Okland it would be an honor if you shared my photos."

Every so often you encounter a patient who reminds you why you love what you do.

There are certain shapes of the nose, lips, jawline that I believe we are biologically programmed to find beautiful. Ratios and curves. But the confidence of being proud of how you look extends well outside the boundaries of my surgery. Confidence, not a shape, should be the surgeon's goal.

My wonderful patient from Nebraska is now 10 weeks out from full face rejuvenation with me. I did a preservation deep plane face and necklift, browlift/reset, functional rhinoplasty with finesse tip-plasty, and full face fat transfer to cheeks, tear troughs, pyriforms, and chin (she also recently had lip filler placed by her injector closer to home).

This surgery was 100% customized to her.

Her primary goals:

1.) Breathe better - she is an avid runner and golfer and had severe nasal breathing including nostril collapse, but cosmetically desired to maintain her strong ‘roman’ nose; slightly deproject and support her tip
2.) Have as crisp a neckline as possible, including improvement in chin projection (fat transfer)
3.) Improve facial volume and look more youthful, but to avoid looking ‘other,’ or ‘different’

Every square centimeter of her face is enjoying a glow-up, and her incisions are nearly invisible at 10 weeks (despite an epic tan).

I love these complex, tailored cases for my patients that combine the highly specialized crafts of rhinoplasty and facial rejuvenation (my two obsessions). This is not factory surgery churning out the same aesthetic over and over again.

This was my first post op patient in the office after spending a week away from work with my family. Reminded me why I love what I do 😀

Revision rhinoplasty glow up 🤯

My gorgeous patient came to me for a tune up after a previous rhinoplasty left her with a heavy, undefined nose (pollybeak deformity). Her nose was also off axis to the right, and she had significant right-sided alar retraction.

Here she is on the table after revision rhinoplasty with me (She also underwent brow-lifting and fat transfer after these photos were taken). She now has a slight slope, clear tip defining point, optimal rotation and projection, and symmetry nostrils with no more retraction 🤓

Will be an incredible glow up for the entire upper and middle third of her face, through only 3 millimeters of skin incisions.

Simulation versus Reality in Rhinoplasty, Volume 2...

During each rhinoplasty consult I work with my patients to simulate a shape they will love and that suits their face/identity. Note that each of these outcomes are different, and each is driven by my patient's aesthetic.

These simulations are my map in the OR, and I work to get as close as possible. Even still, simulations are not a guarantee of result, merely an objective.

Sharing six of my recent rhinoplasty pre-op, simulation, and on the table (plus 1 one month follow-up) results so you can get a sense of the sculpting and nuance that goes into this complex game of millimeters.

At this level rhinoplasty is so much more about your surgeon's eyes than it is their hands.

Primary rhinoplasty with caudal septal deviation and significant nostril asymmetry.

"My nose never fit my face." She was bothered by the hump, her projection, and wanted a more feminine shape at the center of her face.

I noticed significant preoperative nostril asymmetry, caused by her septal cartilage sitting far right of midline. This is one of the few situations where a deviated septum can cause cosmetic AND breathing issues. Caudal ("front") septal deviations can be challenging to correct, because this is a load bearing structure, and if removed (as you normally would a deviated portion of the septum), you could compromise the structure of the nose.

Fortunately I began my career as an Otolaryngologist, which means I have a deep appreciation for nasal function and structure. And I do quite a bit of caudal septal deviation surgery :)

I was able to get her septum back to midline, which dramatically improved her nostril symmetry and, fingers crossed, will improve her nasal breathing as well.

This patient did not want maximum definition or any angularity, so her result is all about shapes. I didn't thin her soft tissue or do any tip grafting. Her nose is softer, more feminine, and looks like something she could have been born with.

Let’s talk about nostril narrowing...

Nostril Narrowing, AKA alar base reductions with sill excisions, AKA alarplasty, is a surgical procedure that allows rotation of the bases of the nostrils inward. This procedure requires precise measurements to avoid asymmetric or too-small nostrils.

I do some version/combination of nostril shortening or narrowing in 75% of my rhinoplasty cases.

However, there are many patients bothered solely by the width of their nostrils, and desire no other changes to their nose.

For these patients I perform in-office nostril narrowing (not rhinoplasty). Following a small amount of local anesthesia, I make discreet incisions along the base of the nostril, remove a small wedge of tissue, and then rotate the nostrils inward. The result is a nose that is less wide at rest and with smile.

Many of my out-of-state patients who undergo nostril narrowing fly in the morning of the procedure and fly out the night of. This is unlike rhinoplasty and facelift patients, who stay a minimum 6-7 days after surgery before flying home.

Nostril narrowing takes less than an hour. As you can see this short procedure can have a profound impact on nasal shape and patient self-confidence.

Primary rhinoplasty with deprojection, nostril narrowing and shortening.

This patient felt her dorsal hump didn't suit her otherwise soft, feminine features. I was able to reduce the hump, deproject her tip but maintain ideal definition. I also spent quite a bit of time harmonizing her nostril length to her new nose.

Selfie Saturday...

This beautiful patient underwent 'preservation' deep plane facelift, revision deep necklift and endoscopic browlift with me. I recently began including the preservation modification in my practice and have loved the early results.

Here she is 3 days out from surgery. Although there's some swelling and bruising this is far less than you might see with a traditional deep plane technique.

So what's different about the preservation modifcation?

Preservation facelift includes the same amount of deep plane dissection, release and lift, but significantly less skin dissection.

In a traditional technique I would separate all neck and lower jawline skin from the underlying muscle. In preservation facelifting far less skin is separated from the muscle (really just under the chin and around the ears). This leads to faster healing, less bruising, and better skin quality.

While it's too early to say whether this modification is superior to the traditional technique long-term, in my hands it clearly accelerates the healing trajectory.

Her result will continue to refine and become more elegant with time.

One of the biggest reasons rhinoplasty has the highest revision rate in cosmetic surgery is that the patients and surgeons cannot read each other’s minds.
 
Said differently, what constitutes a good outcome for rhinoplasty is more subjective. This is unlike virtually any other field in medicine. Rather than science, math, or physics, rhinoplasty is closer to art.
 
And not everyone agrees on art.
 
I’ve oriented much of my practice towards minimizing the distance between what my patients and I think is a great outcome.
 
I visit with my rhinoplasty patients multiple times before surgery. During each visit we use high-quality photography to simulate results – directed chiefly by the patient’s individual aesthetic. They text me inspiration photos so I better understand the curves and definition they consider ideal. I have the simulation photos up on the big screen throughout the case, constantly checking to get as close as possible.
 
And I’m constantly looking for new ways to refine this process.
 
Overall, I’ve found the more time I spend with my patients before surgery, the better decisions I can make for them in the OR, and the more they trust me in their recovery. Each of these compounds in a positive way for my outcomes.
 
“This is exactly how I wanted my nose.”
 
So much is happening behind the scenes to make a patient feel this way. I do not, and cannot, always make this happen. If I could rhinoplasty would be science, not art.
 
My obsession is making this happen as often as possible.
