Dear Patients,
It has been such a great experience helping manage your women’s health needs and any pregnancy related care. I am writing to inform you that this enjoyable experience is unfortunately, coming to an end. Park Avenue Obstetrics & Gynecology, P.C. has permanently closed effective March 21, 2023.
After closing, your health records from this Practice will be transferred to Morgan Records Management, LLC. The Records Custodian will continue to maintain your records in accordance with applicable confidentiality and security standards and with other applicable laws. Your records will be destroyed no less than 21 years after the last date of services you received from this Practice.
After March 21, 2023, you may submit a request to the Records Custodian:

Please Note:
- In accordance with applicable state law, you may be charged for the copying and transmittal of the records.
