Delivering an appliance to a young boy with tongue thrust. The first thing we do is enlarge the upper jaw which in itself improves tongue space and posture, expands airway and creates more room for future incoming permanent teeth. Other factors contributing to tongue thrust are tongue tie, and swallow and speech dysfunction. The tongue tie can be easily addressed with a lingual with the waterlase, and the rest should involve a therapist.

The worst thing you can do to a growing teenager is to extract healthy premolars to make space for orthodontics. The best time to intervene is during the rapid growth phase of ages between six and 10 years old with simple appliances you can develop their arches, and create a healthy foundation for straight teeth, wide smiles, healthy airways, and problem-free jawjoints. This sad case is just an example of how an orthodontist can ruin somebody’s future by not thinking outside the box, being trigger-happy and amputating healthy teeth, just to follow the long-established convention that separates jaw joint problems and airways from straight teeth.

Delivering a lower arch development appliance to a young girl with beautiful eyelashes. The key is early intervention and creating a proper foundation during the rapid growth phase of a child. Many times this will render the need for future braces obsolete or when still necessary, of much shorter duration, avoiding the need for extractions, and ending up with large airways, health jaw joints and a beautiful profile with a wide smile line.

Early intervention in a child ages between 6 and 10 is absolutely necessary and beneficial in treating insufficient jaw size during the rapid growth phase of a child. There are many signs that worn us of future crowding, but one of the earliest is the lateral incisor flaring, rotation, or crowding. If this is not addressed in time, jaw orthopedic therapy becomes much more challenging after age 12 when the primary teeth have been lost and crowding transfers on to the incoming canines and all with a vengeance. This young girl went through two sets of appliances, with the second set, incorporating what we call lap springs to flare the upper central incisors forward, which seemed to have worked wonders, eliminating the need for actual braces.

Restoring a bone level zsystems metalfree same day with an unconventional approach intended to displace keratinized tissue to the buccal aspect of the implant crown. Thanks to the CEREC technology we scanned the abutment while retracting the small gum flap, fabricated the crown and after cementing it, thoroughly debrided excess cement and sutured the flap back. A recap of the and implant procedure follows, showing excellent bone formation using a technique involving both from the patient’s own blood and particulate graft for volume. The patient was very happy to have finished with a total of 2 APPOINTMENTS only.

After a long journey we finished this young man’s treatment. He had four congenitally missing premolars with persisting baby teeth in three of them. The teeth were extracted and during the healing time he straightened his teeth with aligner therapy. Metalfree were placed and restored soon after with Cerec emax crowns. This is his testimony.

The culmination of a long appointment on a young sweet lady who always drives six hours with her husband to receive treatment. Today we placed her second zirconia implant and extracted a wisdom tooth. Herbal relaxants and laughing gas made it all tolerable and almost fun while she watched a documentary on the overhead screen. An important mention here of the significance of a cone beam study in implant planning to know how deep to go with the optimum implant length without hitting the IA nerve. Sure enough I was spot on, stopping less than 1 mm before the nerve as I placed the implant exactly where I wanted.

Most practitioners including myself who quote the ancient Hippocratic Oath of “primum non nocere” (first do no harm) is actually a later Roman simplified version in Latin. Hippocrates is considered the father of Western medicine. He lived on the Greek island of Kos in the 5th century BC. Here are excerpts of his original oath in Ionic Greek, with translation. Ἵππου κήρυκα καλέω Ἀπόλλωνα ἰητρὸν καὶ Ἀσκληπιὸν καὶ Ὑγείαν καὶ Πανάκειαν καὶ θεοὺς πάντας τε καὶ πάσας, ἵστορας ποιεύμενος, ἐπιτελέα ποιήσειν κατὰ δύναμιν καὶ κρίσιν ἐμὴν ὅρκον τε καὶ ξυγγραφὴν τήνδε·. I swear by Apollo the Healer, and Asclepius, and Hygieia, and Panacea, and all the gods and goddesses, making them my witnesses, that I will fulfill according to my ability and judgment this oath and this covenant: Διαιτήμασί τε χρήσομαι ἐπ᾽ ὠφελείῃ καμνόντων κατὰ δύναμιν καὶ κρίσιν ἐμὴν, ἐπὶ δηλήσει δὲ καὶ ἀδικίῃ εἴρξειν.
