Glenhaven, Inc. Glenhaven, Inc. is a campus made up of Glenhaven Rehabilitation and Skilled Nursing, Grand Oaks Assisted Living and Havenwood Assisted Living.

We also have a private hospice suite (made available thanks to Adoray Hospice). Glenhaven, Inc is a nursing home in Glenwood City, WI. There is a 44 resident capacity.
