For over 20 years, AFCNS has helped adults with disabilities live at home with caregivers. Learn about our MassHealth-funded support services.

Exceptional Support and Solutions for Both Caregivers and Clients

Adult Foster Care provides secure private homes and supportive caregivers for adults with disabilities or chronic illnesses. Many Adult Foster Care (AFC) clients are already living with a parent, child or other family member who qualifies as a caregiver. Other clients are placed in homes with diligent and compassionate AFC caregive

rs. If you are an adult with a disability or know of a disabled adult in need of care, AFC may be able to help. Our experienced team of nurses and care managers will assess your needs to see if you or someone you know qualifies for this innovative MassHealth program. Financial Support

Adult Foster Care is a MassHealth-funded program that provides a tax-free daily stipend to the caregiver. Health & Social Support

Our staff members become an extension to the clients' and caregivers' families. Clients and caregivers are assigned a nurse and care manager who visit the homes regularly. Staff members can answer questions about health issues, and serve as a resource for medical training, education and needed interventions. There is always someone on call 24 hours a day, 7 days a week. Family Support

At Adult Foster Care, we provide a foundation to maintain existing relationships. We help our clients navigate the complexities of living with a disability. General Resource Support

Seasoned nurses and care managers work to identify vital resources for the client. These can include:

-Adult Day Health Programs
-Specialized work and community support programs for those with developmental delays
-Mental health counselors
-Guardianship and health care proxy information
-Transportation services
-Information about other adult foster cares in different locations

We get this question all the time: "Can I be the paid caregiver for my disabled son or daughter?"

YES! Parents are some of the most common and dedicated caregivers in our program. 💙
If your child is 16 or older, lives with you, and meets MassHealth and daily care requirements, you can become their official, stipended caregiver through Adult Foster Care of the North Shore.

You have been caring for them their whole life out of love. Through AFCNS, you can now receive a tax-free financial stipend, plus regular at-home support from our Registered Nurses and Care Managers.
(Just a reminder: Under this specific MassHealth program, the person receiving care must be at least 16 years old).

We had such a fun time yesterday sponsoring the Red Sox watch party at the Salisbury Council on Aging! ⚾️

It was a great afternoon connecting with the community, enjoying the game, and sharing some smiles along the way.

🔹 The person receiving care must be 16 or older.
🔹 They must have MassHealth (Standard or CommonHealth).
🔹 They require supervision with at least one daily activity (like bathing, dressing, or eating).
🔹 The caregiver and the person receiving care must live in the same home.
