Adult Foster Care of the North Shore For over 20 years, AFCNS has helped adults with disabilities live at home with caregivers. Learn about our MassHealth-funded support services.

Exceptional Support and Solutions for Both Caregivers and Clients

Adult Foster Care provides secure private homes and supportive caregivers for adults with disabilities or chronic illnesses. Many Adult Foster Care (AFC) clients are already living with a parent, child or other family member who qualifies as a caregiver. Other clients are placed in homes with diligent and compassionate AFC caregivers. If you are an adult with a disability or know of a disabled adult in need of care, AFC may be able to help. Our experienced team of nurses and care managers will assess your needs to see if you or someone you know qualifies for this innovative MassHealth program. Financial Support

Adult Foster Care is a MassHealth-funded program that provides a tax-free daily stipend to the caregiver. Health & Social Support

Our staff members become an extension to the clients' and caregivers' families. Clients and caregivers are assigned a nurse and care manager who visit the homes regularly. Staff members can answer questions about health issues, and serve as a resource for medical training, education and needed interventions. There is always someone on call 24 hours a day, 7 days a week. Family Support

At Adult Foster Care, we provide a foundation to maintain existing relationships. We help our clients navigate the complexities of living with a disability. General Resource Support

Seasoned nurses and care managers work to identify vital resources for the client. These can include:

-Adult Day Health Programs
-Specialized work and community support programs for those with developmental delays
-Mental health counselors
-Guardianship and health care proxy information
-Transportation services
-Information about other adult foster cares in different locations

We’re proud to partner with organizations across Massachusetts to ensure every individual and family gets the support and resources they need. Recently, Adult Foster Care of the North Shore (AFCNS) met with Action, Inc., an agency serving Greater Cape Ann with support in areas such as finance, education, and housing.

AFCNS works closely with community partners to connect families with the right services. When someone needs help, we don’t just point them in the right direction — we walk with them until they get connected.

What Does a Caregiver Do Day-to-Day?

Caregivers play a vital role in helping adults with disabilities or chronic conditions live safely and comfortably at home.

Here are some of the daily responsibilities they support with:

Caregivers are never alone with AFCNS.

Every household receives monthly visits from a registered nurse and a care manager to ensure safety, dignity, and overall wellbeing. In addition to regular check-ins, our team provides 24/7 on-call support and coordinates directly with medical providers to keep care seamless.

We offer training, resources, and continuous guidance — because supporting the caregiver is essential to supporting the participant.

This team-based, whole-person approach ensures that every individual’s physical, emotional, and social needs are met with consistency, compassion, and professional oversight.

Caregiving is valuable — and it should be recognized.

Through the MassHealth Adult Foster Care program, qualified caregivers can receive up to $1700 monthly tax-free stipend for providing daily care at home for a person with a disability or chronic illness.

The care recipient lives in the home of the caregiver and receives assistance with at least one daily living activity (bathing, dressing, walking, toileting or eating.) They must have a MassHealth Standard or equivalent plan and be at least 16 years old.

Examples of usage
-An adult child cares for an aging parent with dementia.
-A person with a physical disability moves in with a non-related friend or caregiver.
-This allows families to focus on stability, comfort, and dignity — not financial stress.

Joyce and Leighan of Adult Foster Care of the North Shore attended Care Dimensions’ Learning Institute Continuing Education presentation, The Emotional Impact of Alzheimer’s Disease on Patients and Families, facilitated by Mary Crowe, Director of Professional and Community Education at Care Dimensions. The event was hosted at Seasons of Danvers, a trusted senior living & memory care community.

Joyce and Leighan both noted the film and subsequent discussion were deeply eye-opening and highly relatable. A key takeaway they noted from the film and subsequent discussion was the importance of honoring the person with dementia by accepting who they are in each moment and meeting them where they are with compassion and understanding.
