Adult Foster Care of the North Shore For over 20 years, AFCNS has helped adults with disabilities live at home with caregivers. Learn about our MassHealth-funded support services.

Exceptional Support and Solutions for Both Caregivers and Clients

Adult Foster Care provides secure private homes and supportive caregivers for adults with disabilities or chronic illnesses. Many Adult Foster Care (AFC) clients are already living with a parent, child or other family member who qualifies as a caregiver. Other clients are placed in homes with diligent and compassionate AFC caregivers. If you are an adult with a disability or know of a disabled adult in need of care, AFC may be able to help. Our experienced team of nurses and care managers will assess your needs to see if you or someone you know qualifies for this innovative MassHealth program. Financial Support

Adult Foster Care is a MassHealth-funded program that provides a tax-free daily stipend to the caregiver. Health & Social Support

Our staff members become an extension to the clients' and caregivers' families. Clients and caregivers are assigned a nurse and care manager who visit the homes regularly. Staff members can answer questions about health issues, and serve as a resource for medical training, education and needed interventions. There is always someone on call 24 hours a day, 7 days a week. Family Support

At Adult Foster Care, we provide a foundation to maintain existing relationships. We help our clients navigate the complexities of living with a disability. General Resource Support

Seasoned nurses and care managers work to identify vital resources for the client. These can include:

-Adult Day Health Programs
-Specialized work and community support programs for those with developmental delays
-Mental health counselors
-Guardianship and health care proxy information
-Transportation services
-Information about other adult foster cares in different locations

AFCNS will be participating in an upcoming community event with the Massachusetts Commission on the Status of Persons with Disabilities and we’d love to see you there!

Join us for Meeting the Moment: A Community Conversation and Resource Fair on Friday, March 27 from 11:00 AM – 2:00 PM at UMass Lowell University Crossing or virtually on Zoom.

This is the second gathering in a statewide conversation series hosted in partnership with MassAbility. The event brings together individuals with disabilities, families, caregivers, advocates, service providers, and community leaders to share experiences, ideas, and solutions that strengthen our communities.

The conversation will focus on important topics including:
• Digital accessibility
• Education and the transition from school to employment
• Workforce supports and development
• Long-term services, supports, and health equity

Two ways to participate:

🗣 Community Conversation (11:30 AM – 12:30 PM)
Community members are invited to share remarks. If you’d like to speak, written remarks must be submitted in advance by March 23 to cspwdinfo@mass.gov

Emotional and mental well-being are just as important as physical health. Staying socially connected helps older adults and individuals with disabilities feel engaged, valued, and supported in their daily lives. Building relationships with friends, neighbors, and family strengthens overall wellness and fosters a sense of belonging.

Ways to support mental health and social connection:
• Regular phone or video calls with loved ones
• Participating in hobby groups, book clubs, or craft circles
• Attending local senior or community centers that host social activities
• Following routines that promote relaxation and stress relief, such as journaling or meditation
• Learning about healthy aging and emotional wellness

Caregivers and family members play a crucial role by:
• Scheduling and encouraging participation in activities that bring joy
• Helping residents maintain regular social interactions
• Watching for signs of loneliness, withdrawal, or mood changes
• Offering simple gestures, like weekly check-ins or arranging transportation to events

In this way individuals with disabilities can enjoy meaningful connections, reduced isolation, and improved mental well-being — all while living safely and confidently in a supportive, home-like environment. 💙

❤️ Spreading the Love in Amesbury ❤️

AFCNS was happy to deliver Valentine goody bags to the Amesbury Senior Center, where Courtney Hutchinson and Stephanie Caron graciously accepted them on behalf of their team.

These bags will be included with Meals on Wheels deliveries and shared through other senior center programs — a small gesture to help brighten someone’s day.

We’re so grateful for our community partners and the meaningful work they do every day to support older adults. It’s an honor to work alongside you.
