PATCH Coalition of Taylor County We are an independent 501(c)(3) nonprofit organization that operates solely on donations and project-based grants.

The goal of the Planned Approach To Community Health (PATCH) Coalition of Taylor County is to improve the over health and well-being of Taylor County and its citizens through education and special projects. The Planned Approach to Community Health (PATCH) Coalition of Taylor County was organized in 1993 as a partnership between the Centers for Disease Control (CDC), the West Virginia Bureau of Pub

lic Health, a group of concerned citizens and health professionals in Taylor County. We have tax exempt status as a non-profit organization since July 1998. Our mailing address is PO Box 304, Grafton, WV 26354. Our areas of concern include the seven dimensionsof wellness: physical, emotional, social, intellectual, spiritual, environmental, and occupational. Our special ongoing projects involve community access to menstrual products (Project Period for Taylor County) and general hygiene products (Blessing Box Hygiene Project).
