PATCH Coalition of Taylor County We are an independent 501(c)(3) nonprofit organization that operates solely on donations and project-based grants.

The goal of the Planned Approach To Community Health (PATCH) Coalition of Taylor County is to improve the over health and well-being of Taylor County and its citizens through education and special projects. The Planned Approach to Community Health (PATCH) Coalition of Taylor County was organized in 1993 as a partnership between the Centers for Disease Control (CDC), the West Virginia Bureau of Public Health, a group of concerned citizens and health professionals in Taylor County. We have tax exempt status as a non-profit organization since July 1998. Our mailing address is PO Box 304, Grafton, WV 26354. Our areas of concern include the seven dimensionsof wellness: physical, emotional, social, intellectual, spiritual, environmental, and occupational. Our special ongoing projects involve community access to menstrual products (Project Period for Taylor County) and general hygiene products (Blessing Box Hygiene Project). Taylor County Schools will be offering Multiday Meal Boxes as part of the 2025 Summer Feeding Program. Meal Box information is provided in the attached form.
