At Partnership Community Health Center (PCHC), our mission is to make a difference in the lives of people and create a healthier community.
Since our humble beginnings as a free clinic in the basement of the Salvation Army over 20 years ago, we have grown into a free-standing Federally Qualified Health Center (FQHC) that serves over 16,000 patients a year. We are proud to offer primary medical, dental as well as integrated behavioral health care regardless of ability to pay. We accept BadgerCare/Medicaid, Medicare, most commercial insurances and offer nearly all of our services on a Sliding Fee Scale for those who are uninsured. As a complement to our clinical services, we also provide free in-person health insurance enrollment assistance and link patients with 340B pharmacy services to help reduce out-of-pocket prescription costs.
Currently, we have locations in Appleton, Oshkosh, and Waupaca. We welcome everyone to come experience our compassionate, patient-centered approach to care.
See you soon!
