I am often asked by my clients for advice on what they can do to prevent tearing during childbirth. While nothing can give a 100% promise of no tearing, here are six approaches that may help reduce the risk of tearing...

1️⃣ Listen to Your Body
Allowing your body to guide the pushing process, rather than forceful or prolonged pushing, can help minimize perineal stress.​

2️⃣ Controlled Breathing During Pushing
Using deep abdominal breaths and open-glottis pushing during labor can help shorten the pushing phase and reduce the need for medical interventions, leading to a smoother birth.

3️⃣ Avoid Routine Episiotomy
Unless medically necessary, avoiding routine episiotomies may reduce the risk of more severe tearing.

4️⃣ Perineal Massage
Starting around 34–35 weeks, a gentle perineal massage can increase tissue flexibility. Some studies suggest it may lower the risk of episiotomy and reduce postpartum pain, especially for first-time moms.

5️⃣ Warm Compresses During Crowning
Applying a warm compress to the perineum during the second stage of labor has been associated with a reduction in second- and third-degree tears and decreased pain intensity. ​

6️⃣ Optimal Birthing Positions
Positions like side-lying or upright may help reduce pressure on the perineum, potentially lowering the risk of tearing.

Remember, every birth is unique. These strategies may help, but it's essential to discuss them with your healthcare provider to determine what's best for you.
