The Sawtooth Mountain Clinic is looking for a competent and caring Registered Nurse to join the Nursing Team as a Care Coordinator. The Health Care Home (HCH) Care Coordinator collaborates with primary care physicians, clinic staff and other Health Care Home team members to identify needs, provide education, and coordinate plans of care for identified patients. The care coordinator assists patients to achieve health goals and improved health care outcomes. The Care Coordinator is also an advocate for patients and, depending on patient need, may link patients with health and community resources that provide a range of services, promote self-management, improve health and reduce disparity.

Salary: Starting at $35/hour commensurable with education and experience.

This week we are celebrating National Health Center Week, a celebration of America's Health Centers and their 60-year legacy of providing high-quality, affordable, and accessible care to millions.

Today is Staff Appreciation Day and we are thankful for our dedicated staff everyday for all they do for our patients! This past weekend many of our staff volunteered their time to take part in the Fish Pic fun. SMC staff helped out at the aid station at the Pincushion Trail Run/Walk and ran the bingo tent to raise money for the Giving Tree. The Oral Health Task Force took part in the parade and passed out toothbrushes and toothpaste!

The Sawtooth Mountain Clinic is looking for a friendly, hardworking, and detail-oriented person to fill the role of Floor Nurse - This person may be an LPN, a Medical Assistant (MA), or someone looking to gain their MA Certificate - no prior experience necessary! MA Apprentices learn on-the-job and receive free medical assistant education while working full-time, getting paid, and receiving a full benefit package!

Salary: MA Apprentice starts at $18.50/hour with increases for apprentice milestones, other certifications will be paid commensurately with educational background and experience.
