🤰**"Do I really need PT during pregnancy?"**
We hear this question all the time — and here's the scoop:

Some women feel wonderful during pregnancy and also benefit from our Pregnancy PT and Push Prep Program. This personalized treatment program specifically identifies and teaches each woman how to push, what positions work better for her and how to optimize her perineum to reduce risk for tearing. We also work throughout the pregnancy to optimize the woman's ribs, hips, pelvis, pelvic floor, low back and abdomen so that motor control & strength are ideal and the mom's body provides the baby the space necessary to position itself head-down and in optimal position for birth can be achieved. We use manual therapies, rehabilitative ultrasound imaging, education, perineal massage, heart rate variability training and therapeutic exercises personalized to each person.

Additionally, research has found that the presence of pelvic floor dysfunction is common, even in adolescent athletes. Therefore, when pregnant, often underlying pelvic floor tension, weakness or dysfunction are often present. Treating these can help your baby position better, your body feel better and your birth go more smoothly. Yet, this is only one small part of what our Pregnancy PT addresses.

Pregnancy changes everything about your body — how you move, how you breathe, and how you carry your growing baby. When pregnant often women experience neck & back pain, carpal tunnel syndrome, pelvic floor tension, bladder leakage, painful hips when rolling in bed, core weakness, round ligament pain, lumbopelvic pain (p***c pain, tailbone pain, SI joint pain, hip pain, etc) and other musculoskeletal issues. So often back problems, hip issues and pelvic floor dysfunction are frustrating to improve.

The inner thighs, with their attachments to the pelvic bone and their role in stabilizing the pelvis with each step we take, play an important role in chronic back pain, hip pain and pelvic floor dysfunction.

Often our patients, before coming to see us, have already tried to improve their back, pelvic floor or hip problems, yet remain with bothersome symptoms.

We are the only specialty pelvic health, spine and TMJ clinic in West Michigan. Our PTs implement a unique whole-body framework when designing a rehabilitation program. It is our goal to provide patients a trauma-informed, personalized and holistic treatment plan that yields durable results.

Some common things we identify that are often overlooked:
🟪 inner thigh weakness, tightness, overuse or facial restrictions
🟪Pelvic floor dysfunction that has been overlooked and is contributing to back pain & hip pain
🟪Visceral & abdominal muscle contributions to ongoing pelvic floor, back and hip dysfunction
🟪Scar tissue, especially in abdominal surgery scars, causing pelvic floor, lower back and hip problems
🟪Muscle imbalances most notable in weakness of hamstrings, inner thighs, hip rotators and glutes and compensations or overuse of hip flexors, pelvic floor and sometimes inner thighs.
🟪Ribcage & thoracic spine dysfunction, especially stiffness and impaired diaphragmatic capacity and function contributing to ongoing pelvic floor, spine and hip issues.
🟪Postural and movement pattern dysfunction caused by various points of weakness, fascial restrictions, deep core deficiency and habits. These contribute to ongoing back, pelvic floor and hip problems.

Can we honor and celebrate Women and our contribution to making the world go round?

Womens health, we all are aware, has been an after thought in NIH funding, decades of research and in our sociological framework.

The truth is that women experience greater risk of life long problems related to pelvic health injuries. These problems reduce our quality of life and can impair our confidence and ability to stay fit and active.

We also, often, neglect to commit to working out, because we are taking care of everyone & everything else.

It’s crucial to have our bodies functioning at top form.

What are some things you can do for yourself?
☘️ Prioritize your mind, body and spirit with preparation during pregnancy, including push prep, pelvic floor care, and abdominal wall & spine health
☘️ Be committed to postpartum rehabilitation. Don’t just think it will all magically get back perfectly fine. You need to do the work.
☘️Know your menstrual cycle, thyroid, sleep quality & glucose levels. Be proactive with optimizing and fixing all of these. Don’t hesitate.
☘️Perimenopause & menopause exacerbate pelvic floor, bladder, bowel & musculoskeletal problems. See your pelvic PT and take action.
☘️ Maintain ideal body weight by eating for nourishment and developing muscles, mobility & cardiovascular health.
☘️Jump and move quickly at every age, so you build bone and reaction time to reduce risk for falls and fracture.
☘️See your pelvic health PT annually for a check up & commit to tune-ups with PT as soon as symptoms arise.

My own mother recently told a joke (not a joke) that “very few people, except moms, understand the impact of driving a truck through your pelvic floor” (in reference to giving birth).

We know that people who have back pain tend to be uncertain about what to do. Patients often ask us whether they should rest and stop activities that bother their back or should they continue the activities. Patients also tell us that they always get muscle spasms and back stiffness & can’t figure out how to get these to stop.

So much is happening to our bodies when pregnant. It’s important to help the hips, pelvis, abdominals, rib cage, & pelvic floor during pregnancy. We need a public health campaign to teach women what they can do to build their bones, to reduce bone loss and to get stronger bones if they have osteopenia or osteoporosis.

Basic info on building bones when menopausal or premenopausal:
🌟 jumping helps build bone strength🌟Multi-directional jumps can confer greater benefits than doing the same thing over and over
🌟Jogging may not be enough of a stimulus to get the results you need
🌟Jumping up, such as onto a box or stair, may help some women, when done safely and correctly.
🌟Drop jumps (such as hanging from monkey or pull up bars and letting go) have been shown to build bones. But these are huge force impact and hard on the joints, so not right for a lot of beginners.
🌟Mini-tramp does not seem to provide the necessary ground reaction force stimulus to your bones to improve bone density.
🌟Wrists & arms get osteoporosis, also, so weight bearing on these is important
🌟Anyone who has a lower extremity fracture in adulthood needs to assume they lost bone mass when recovering. The first 12 weeks you are at risk for another fracture, such as a stress fracture.
🌟Too much training when you are not ready for it or when you don’t give yourself adequate recovery, leads to injury.
🌟Learning how to jump to build bones is different than jumping with soft landing. It’s really important to know how to do this safely.
🌟Three, 10 minute sessions a week of jumping has been found to improve bone density.

We help women through the lifespan including perimenopause & menopause to build bones, get strong, address pelvic floor & core issues, improve balance to reduce fall risk. Prolapse is such a difficult thing to live with and our patients experience fear of making it worse.

If you have a prolapse, did you know that if you had obstetric trauma including
✅a high birth weight child
✅Episiotomy
✅Perineal tearing or laceration
✅Forceps delivery
✅Vacuum extraction

That you are higher risk for having a pelvic organ prolapse? AND that research has found you are likely to benefit room pelvic floor PT, including pelvic floor muscle training.
