We bring the clinic to you! Female therapist owned and operated, fully licensed and insured, and HIPAA compliant service. Our team has extensive experience in therapy, including multiple settings. Home Connection Therapy offers a personalized therapy experience and our patient's are like family! You chose the direction of your therapy sessions with personalized care plans based on your goals. We bill directly to your health insurance and also have wellness / private pay options available. Our therapists come to you, to help maintain your independence and continue to thrive in your own home! Evaluations typically within 24 to 48 hours. Services include:
✨ Home safety assessments: home modification and medical equipment (DME) recommendations / ordering
✨ Activities of daily living retraining / modification
✨ Home management retraining / modification
✨ Functional mobility and transfer training
✨ Fall prevention / management - Certified Fall Prevention Specialist
✨ Functional balance rehabilitation
✨ Dementia care: caregiver education, activity modification
✨ Rehabilitation following orthopedic surgery / joint replacement
✨ Rehabilitation following stroke or other neurological impairments - ALS, MS, Parkinson's disease
✨Medication management education
✨ Transition planning recommendations (appropriate level of care)
✨ Post COVID rehabilitation
✨ Home exercise program establishment

*No hospital stay required, no need to be "homebound", no referral / order required (our therapist's will speak to your doctor to obtain if needed)
