United Rehab Consultants is an independent case management company, specializing in auto injury, private pay and workers compensation cases. An important role of case management is helping to ease the burden of managing care and assisting with the process of recovery. Case manager duties include assessing, planning, implementing,monitoring and evaluating actions required to meet the client's goals and treatment plan. Must be a RN, LPN, OT or Social worker with strong medical background.
Position includes flexible schedule, ability to work remote, some travel is required to attend physician appointments, home evaluations, and hospital meetings, etc.
Job Responsibilities:
Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered.
Handle case assignments, document daily interactions, write reports, review case progress and determine case closure.
Help clients achieve wellness and autonomy.
Facilitate multiple care aspects (case coordination, information sharing, etc.).
Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options.
Develop effective working relations and cooperate with medical team throughout the entire case management process.
Take the extra mile and interact with Clients to keep track of their progress and to ensure satisfaction.
Record cases information complete all necessary forms and produce reports accurately and in a timely manner.
Promote quality and cost-effective interventions and outcomes.
Assess and address motivational and psycho-social issues.
Adhere to professional standards as outlined by protocols, rules and regulations.
Requirements
Must have a Nursing, Social Work, Occupational or Physical therapist degree.
Must be familiar with computers: Microsoft Word, PDFs, internet use and emailing.
Previous case management and/or home care experience preferred.

Reliable transportation for work related travel.
Other tasks and responsibilities as delegated by manager.
