Older adults with a standard Medicare Plan and a diagnosis of Dementia are eligible to receive $2,563 a year to spend on Respite care for their caregiver. This benefit can be used in an Assisted Living Facility for companion care or to fill in when the older individual requires more specific, even if temporary, attention.

A Matter of Balance (MOB) is an evidence-based, award-winning falls prevention program designed for older adults who are concerned about falling. Through 8 two-hour sessions led by trained coaches, participants learn to view falls as controllable, increase their physical activity levels, identify fall hazards in their homes, and perform exercises to improve balance and strength. The goal is to empower older adults to reduce their fear of falling, gain confidence, and improve their overall physical well-being.

What is the “A Matter of Balance®” Program?

Help older adults reduce their fear of falling by learning to view falls as controllable. A Matter of Balance will also set goals for increasing activity levels, encourage small changes to reduce fall risks at home, and ways to exercise to increase strength and balance. A Matter of Balance is a program led by a certified trained coach and is specifically designed to reduce the fear of falling and improve activity levels among community-dwelling older adults. The program includes eight two-hour classes presented to a small group of 8-12 participants led by trained coaches.

For many months we've officially had over 115 caregivers on our roster. Sometimes caregivers are off during any given week for a variety of reasons. This week, for the first time, 100 caregivers worked shifts with folks in the community, providing 1800 hours of care to over 200 people.

Ensuring high quality care reaches all individuals with dementia is a crucial aspect of the GUIDE Model.

Dementia imposes significant financial, emotional, and logistical burdens on families.

The GUIDE Model offers a variety of financial and technical supports to ensure that participating safety-net providers can develop their infrastructure, improve their care delivery capabilities, and participate successfully in the model.

GUIDE also includes a focus on people with dementia who are dually eligible for Medicare and Medicaid and, as with other people supported by the model, help them to remain safely in their homes for longer.
