We’re honored to be attending this year’s event in October.

For us, it’s not just about growth conversations. It’s about connection.

We’re looking forward to meeting Adult Foster Care owners and leaders who carry so much responsibility for their residents and families — especially during life’s most tender and uncertain moments.

Hospice is never an easy conversation. But when the time comes, having the right support makes all the difference. Our team is committed to walking alongside AFC homes with compassion, clarity, and respect — ensuring residents are comfortable, families feel heard, and owners feel supported every step of the way.

If you’re attending, we’d love to connect, listen, and learn how we can better serve your community together.

See you in October. 💚

How do you know you’ve outgrown survival mode?

- It’s not when your beds are full.
- It’s not when inspections stop stealing your sleep.

It’s when you’re ready to replace hustle with systems.

You’ve built something real.
You’ve made the hard calls.
You’ve learned the lessons the hard way.

Now you don’t need motivation, you need leverage.

That’s what the Scaling Up Conference is built for.

This isn’t an inspirational event. It’s a working room for operators ready to stop carrying everything themselves and start building businesses that scale.

In two focused days, you’ll work through what actually moves the needle:
• Hiring and retaining staff without chaos
• Knowing what to charge — and why
• Preparing for audits before they become emergencies
• Using AI and technology without losing the human side of care
• Knowing when (and how) to expand

Plus breakout sessions led by leaders shaping the future of AFC care — covering dementia communication, LARA insight, non-profit practices, CMH & Recipient Rights collaboration, HR best practices, investment and growth strategies, and MORE!

And you won’t do it alone.

Real experts. Real solutions. Real connection.

You walk in with questions.
You leave with clarity, strategy, and momentum.

If you’re ready to move from holding it together to intentionally building what’s next — this is your room.

💚 | Bryan Cramer
Executive Director, Byron Manor | Leader | Culture-Driven Advocate

Bryan Cramer’s path into healthcare began with a deeply personal experience. In 2005, after both of his grandparents passed away with hospice care in New York—his original home state—Bryan felt a calling to serve others during life’s most vulnerable moments. That calling brought him to West Michigan and into community-based hospice work, where he quickly realized healthcare wasn’t just a career choice—it was where he belonged.

During his early years in hospice, Bryan immersed himself fully in the field. He went on to earn his Bachelor of Science in Healthcare Administration with a 3.89 GPA, while naturally stepping into leadership roles. He found himself guiding, coaching, and encouraging those around him to do the right thing—always with passion, honesty, a positive attitude, and a genuine smile.

Now 21 years into a career dedicated to hospice care, Bryan has spent the last five years serving as Executive Director of Byron Manor. His leadership is grounded in three core values: culture, consistency, and communication. Bryan believes exceptional care starts with strong teams and clear connection—and he leads with that belief every day.

For Bryan, this work is personal. He doesn’t see Byron Manor as a facility—it’s home to the residents he serves. “If they aren’t comfortable, I’m not comfortable,” he says. That mindset drives his approach to leadership and care, ensuring dignity, comfort, and humanity are never compromised. He takes pride in finding solutions, no matter the challenge, because comfort and trust matter.

Hospitality is at the heart of who Bryan is. He genuinely enjoys making people smile, laugh, and feel seen. Brightening someone’s day—whether a resident, family member, or team member—isn’t extra; it’s essential.

Outside of work, Bryan is a devoted family man. He and his wife, Jen—whom he’s known since the start of his healthcare journey—have called Hudsonville home for the past 11 years. Together, they’re raising their two children, Lennon (2) and Lewis (6). Life beyond work includes home remodeling projects, spring and summer landscaping, and family joy rides in their Bronco with the top off, soundtracked by Jelly Roll, Phil Collins, the Beach Boys, or Nelly.

Bryan also makes it a tradition to travel with friends to a different MLB stadium each year, taking in the energy, culture, and stories of each city.

As his grandpa Pete always used to say:
“Nothing but fun all the time!”

💚 | Dylan Hescott
Business Development Manager | Senior Care Advocate | Relationship-Driven Leader

As the senior population continues to grow, families are asking for more than availability—they’re seeking quality, consistency, and care that feels deeply personal. In an industry expanding at record speed, the true challenge lies in ensuring growth never outpaces the human connection at the heart of care.

That belief has guided Dylan Hescott’s decade-long journey in senior healthcare.

Raised in a small town in Northern Michigan, Dylan grew up with a strong sense of community—an influence that continues to shape both his leadership style and his approach to care. He earned a Bachelor of Science in Biomedical Sciences from Grand Valley State University, initially intending to pursue physical therapy. But it was through volunteering at a local retirement community that his path shifted. What started as service quickly became a calling: supporting older adults in ways that honored dignity, individuality, and trust.

That calling led Dylan to Acti-Kare of Grand Rapids, where he connected with owner Kevin Kampfschulte and embraced a people-first philosophy rooted in caregiver empowerment, intentional caregiver–client matching, and truly individualized care. Over the next seven years, Dylan played a key role in helping establish Acti-Kare as a trusted home care provider across West Michigan—proving that quality and growth don’t have to compete.

Following the pandemic, Dylan expanded his impact into hospice business development, stepping into work that required both strategic thinking and emotional presence. For three years, he walked alongside families during some of life’s most vulnerable moments, gaining a deeper understanding of what compassionate, end-of-life care truly requires. That chapter sharpened his perspective and reaffirmed his purpose.

Today, Dylan has returned to Acti-Kare with renewed clarity and conviction—focused on advancing home care in a way where relationships remain central and growth never compromises quality. Bringing his journey full circle, he continues the volunteer work that first inspired him, now serving at a local retirement community through the SAIDO Learning program, engaging residents living with dementia and reinforcing his belief that connection, dignity, and presence matter at every stage of life.

For Dylan, impact isn’t measured solely in metrics or milestones—it’s found in consistency, trust, and showing up with intention for both clients and caregivers alike.
