This patient started noticing the changes so many patients describe first; softening along the jawline, early jowling, and laxity in the neck. For her, the goal was to just restore her tissues to their natural position before the early aging process started.

She underwent a deep plane face & neck lift combined with an endoscopic brow lift to reposition the underlying support layers of the face rather than simply tightening skin. This allows the cheeks, jawline, and neck to be lifted in a more anatomical way and the brow to be elevated for a brighter, rested appearance (also helps with any laxity in the upper eyelids!)

This is just 1 day post-op. Swelling is still very present and the tissues are early in healing, but you can already appreciate the improved contour through the jawline and neck. Results will continue to refine over the coming weeks as swelling resolves and the tissues settle into their final position, this is just the beginning of her transformation.

A recessed chin can soften the jawline, shorten the lower face, and make the profile appear less defined even when everything else is proportionate. By placing a medium silicone chin implant, we were able to bring the chin forward, sharpen the jawline, and improve overall facial harmony.

Chin augmentation is a permanent solution that restores proportion to the lower third of the face and often enhances the appearance of the neck and profile at the same time. Many patients are surprised how much definition it creates without needing fillers or repeated treatments.

After significant weight loss, one of the most common concerns patients experience is excess skin of the upper arms. While weight loss improves health and body composition, the skin often does not have the elasticity to retract, leaving hanging tissue that cannot be corrected with exercise alone.

A brachioplasty (arm lift) is designed to remove that excess skin and reshape the contour of the upper arm. Unlike procedures such as a tummy tuck where the incision can be hidden low on the abdomen, the arm lift incision does extend along the inner arm and is not completely concealed. This is an important part of the consultation discussion, we always prioritize informed decisions and realistic expectations.

However, for many patients, the trade-off is worth it. The improvement in arm contour, clothing fit, comfort, and confidence often far outweighs the visibility of the scar. Over time, the incision continues to mature and fade, while the contour improvement remains.

Major Thigh Transformation! 🦵💫

This patient struggled with significant skin laxity throughout the thighs post major weight loss, which ended up requiring more than one procedure to fully address. We began with an extended thigh lift to remove excess skin through the mid to lower thighs, improving contour, smoothness, and overall shape.

After healing and allowing the tissues to settle, she returned a year later for a bilateral circumferential thigh lift. This second stage focused on the upper inner and upper posterior thighs, targeting remaining skin redundancy.

Staged surgery like this allows us to be strategic, see what areas still need improved contour step by step while protecting blood supply, optimizing healing, and achieving a more comprehensive, natural-looking outcome.

After massive weight loss, we created a strategic surgical plan designed to help this patient reach her ultimate goals not just physically, but in her confidence too. Transformations like this don’t happen overnight; they happen with careful planning, healing, and trust in the process.

Her journey began with an extended tummy tuck using a fleur-de-lis approach to address upper abdominal skin laxity that a standard tummy tuck alone couldn’t correct, along with a breast reduction. CO₂ Laser: Part 2 Healing & What to Expect ✨

Because CO₂ laser is a fully ablative treatment, the healing process is just as intentional as the results. In the first few days, swelling, redness, and a sunburn-like sensation are completely normal as your skin begins its renewal process.

By weeks 3–6, redness continues to fade while collagen remodeling is actively taking place beneath the surface. Improvement doesn’t stop there, collagen production continues for months, meaning your results keep getting better with time.

Let’s talk about fully ablative CO₂ laser resurfacing, one of the most powerful skin-rejuvenation treatments available.

CO₂ laser works by removing the outer layers of damaged skin while stimulating deep collagen remodeling, helping improve wrinkles, etched-in lines, texture, sun damage, and overall skin quality. Because this treatment is fully ablative and highly intensive, it is most commonly performed in the operating room, often in combination with procedures such as a facelift or as a stand-alone treatment when appropriate.

Unlike lighter resurfacing treatments, fully ablative CO₂ laser is typically designed to be done once, rather than as a recurring maintenance procedure, due to its depth, power, and long-lasting impact on the skin.

About Dr. Rudy Coscia

Dr. Coscia began practicing at The Plastic Surgery Center in 2012 as a cosmetic fellow. He quickly earned the respect and praise of the senior surgeons and was asked to join the group in 2013. Dr. Coscia has developed a reputation of having superb surgical skills and a wonderful demeanor. Dr. Coscia is also board certified by The American Board of Plastic Surgery. In addition, he is a member of The American Society of Plastic Surgeons and The California Society of Plastic Surgeons. Dr. Coscia received his medical degree in his hometown of Memphis at the University of Tennessee. There he had the unique opportunity to work in the trauma department as a suture tech. The experience of caring for trauma patients and working to correct severe traumatic facial injuries sparked his initial interest in plastic surgery. Visualizing the problem and working to obtain the desired results was exciting and rewarding. During medical school he was selected for membership in Alpha Omega Alpha, an honor reserved for the top students.

Dr. Coscia was drawn to Northern California by the beautiful weather and vast array of outdoor activities. Here he completed his surgical training at the University of California Davis Medical Center where he worked under Dr. Thomas Stevenson, a nationally recognized leader in plastic surgery. Upon completing his residency, Dr. Coscia served as the fellow for The Plastic Surgery Center under the direction of six board certified plastic surgeons. During this fellowship year, he acquired tremendous experience performing facelifts, rhinoplasties, breast augmentations and breastlifts, and abdominoplasties. Dr. Coscia has a special interest in facial plastic surgery but appreciates the broad scope of plastic surgery both cosmetic and reconstructive.

Dr. Coscia is a published author with articles appearing in the Annals of Plastic Surgery. He has presented his research at meetings such as the California Society of Plastic Surgeons and the Aesthetic Society of Plastic Surgeons. He is a candidate member of the American Society of Plastic Surgeons, the California Society of Plastic Surgeons, and the American Society of Aesthetic Plastic Surgeons.

He now has made Sacramento his home but enjoys traveling and visiting his family. Backpacking and skiing are two of his favorite pastimes. He also loves music and often sings at his church in Rocklin. He feels blessed to live in an area with so much to offer and privileged to be part of a practice with such a long and respected reputation.
