Last November on Mother Knows Death Podcast, we talked about an insane colonoscopy finding!

A 63-year-old male went in for a screening colonoscopy that was requested by his primary care physician. He had taken an adequate bowel prep and his doctor was easily able to visualize his colon. During the procedure, it was noted that he had diverticulosis, which are little pockets present in the bowel wall. Diverticulosis itself can cause problems because food can get stuck in these little pockets and cause inflammation. Once the pocket is inflamed it is now called diverticulitis. Diverticulitis can cause pain and bleeding as well as potential other problems like perforation (a hole in the colon).

While the doctor was examining the transverse colon, they saw something very unusual. AN INTACT FLY!

After the patient woke up from the procedure the doctor asked him how the fly got into his colon.

Duh, of course, he could not explain it. He said he drank clear liquids as part of the prep and the night before he had pizza for dinner. He did not recall eating a fly.

This finding is really confusing because the fly was halfway through the colon and completely intact. So, how did it make it though through the hydrochloric acid in the stomach!!??

Well, believe it or not, there have been cases of insects making their way intact through the GI tract so it is possible!

I have a couple of theories, let me know what you think!

1. A fly was in the clear liquids he was drinking for the procedure. Maybe since he was on a fast, the stomach was not producing as much acid since there was not really food to break down.

2. The patient accidentally swallowed maggots or fly eggs on food the last time he ate. They survived the stomach and got stuck in one of the diverticula and grew into a fly.

3. A fly was present in the GI suit and entered the colon at the time of colonoscopy

Conjoined twins spontaneously aborted at 14 weeks of gestation!!

This is a case of showing conjoined twins fused at the abdomen (omphalopagus), which means the union in a transverse plane in the lower body area, so that the twins face each other.
Bare in mind miscarriage is a naturally occurring event due to complications and has nothing to do with medical or surgical abortions, or any interruption of the pregnancy!

In broad terms, conjoined twins can be regarded as a doubling anomaly. The later the incomplete embryologic separation occurs, the higher the likelihood of a complicated fusion.
It more precisely refers to an incomplete splitting of the monozygotic twins after 12 days of embryogenesis, ending with the fetuses being physically joined at some point.
That point of union is used to classify twins.
This is a case of a omphalopagus presentation, where the anterior abdomens are united combined with the union of a small portion of the lower chest. This can present with or without a cardiac union.
These twins share a common abdominal wall and may have varying degrees of fusion involving abdominal organs such as the intestines, liver, and other structures in the abdominal region. Dr.Rana's Sagar Jung Rana

A generalized tonic–clonic seizure, or GTCS, previously known as a grand mal seizure, is a type of generalized seizure that produces bilateral, convulsive tonic and then clonic muscle contractions. Tonic-clonic seizures are the seizure type most commonly associated with epilepsy and metabolic imbalances. The seizure itself includes tonic (sustained) and then clonic (repetitive short) contractions. For a person experiencing a tonic–clonic seizure, first-aid treatment includes rolling the person over into the recovery position, which can prevent asphyxiation by preventing fluid from entering the airway. Long-term therapy may include the use of antiepileptic drugs, deep brain stimulation surgeries and more.🧠

This is a case of 57 year old male who was apparently alright but he suddenly developed chest pain while working out and became unconscious. He was then brought to our hospital and declared brought dead and the body was sent for post-mortem examination.

On autopsy, there was e/o 130 ml of blood clot in pericardial cavity surrounding the heart.

On further examination, there was rupture of inferolateral (posterior) wall segment of left ventricle.

On further dissection, there were areas of infarcted myocardium with thinning and fibrosis of left ventricular wall at the rupture site.

Final opinion regarding cause of death: Cardiac tamponade due to rupture of left ventricular wall.
