One thing I love most about plastic surgery is that thinking outside the box is inherent to our profession.
At times we have to think outside of a two dimensional plane to create a three dimensional structure.
This is one technique I commonly use to reconstruct a nipple, if it had to be removed for breast cancer.
This design is called a C to V flap. We cut the skin in a pattern that will be formed into a semblance of a nipple. The blood supply that keeps the skin alive flows through the part that we did not cut. Next the skin gets stitched and coned to the shape of a nipple.
Several months later, once the skin is healed, we tattoo the surrounding area to create a natural appearing areola. Modern techniques of medical tattooing help generate a very natural appearing nipple areolar complex.

Angela Restoried from Restored
When Anglea and her husband got married, they knew they wanted children. Angela struggled with fibroids which made it extremely difficult to conceive, however, after years of trying, she had a daughter via IVF. A month after her daughter was born, her doctor found a lump in her breast.
She was diagnosed with ductal carcinoma in situ for which she chose to undergo bilateral mastectomy and implant-based breast reconstruction.
“When I was diagnosed, I was so scared, sad and depressed at the same time. I was like, ‘this is going to change my life’” she said.
Her life did change: not long after recovering from her operations, she became pregnant with her second child. Changes in her body after the birth of her son made her choose to have her implant-based breast reconstruction converted to DIEP flaps.
“After that surgery, I just felt like my old self again, which I felt like I’d been looking for for two years. I was able to feel so good after my surgery.”

Donna paid me a visit yesterday, five years after she paid it forward by agreeing to participate in #ProjectReconstructed, in which we painted the scars of breast reconstruction in gold, inspired by #kintsugi, the art form that embraces the philosophy of viewing the beauty in repair.
Donna didn’t know that in the subsequent years, she has inspired many women, amongst them Jocelyn, to embark on the journey of dealing with a cancer diagnosis with full hearts and strong smiles.
Yesterday I gave her our book #Restored, featuring 12 such women, including Jocelyn @thebrcababe, in a collaboration with @formcollaborative @erezsabag @davidwarren01 @dr.ronisraeli of @nybra_plastic_surgery. Here we cast, sculpted, and photographed the women, creating tangible evidence of their resilience.

Behind the scenes footage of me trying to find the precise moment where I gave up on the tie on yet another weekend morning, working on a new textbook on complex breast reconstruction.
The premise of the book is to extract tips and tricks from experts across the country, in a non-traditional manner. Scripted lectures at conferences and grey review chapters (often delegated to junior residents…) serve their didactic role. If you really want to get the pearls and pitfalls, talk to the tipsy professor at the bar after the meeting’s day schedule is over.
I don’t really drink (Blanton’s Old Fashioned, extra bitter), so this is my way of having a go at it.
