Loretta, 60, with a complex medical history that included super morbid obesity, lupus, restless leg syndrome, chronic pain, and obstructive sleep apnea with obesity hypoventilation syndrome, experienced a significant health setback that greatly impacted her independence and quality of life. She was admitted to Kindred Hospital North Florida with worsening shortness of breath, significant lower extremity swelling, and pain. Upon admission, Loretta required high-flow oxygen support and demonstrated a marked decline in her ability to perform activities of daily living.

Recognizing the complexity of her condition, an interdisciplinary team developed a personalized rehabilitation plan led by physical and occupational therapy. Initial goals focused on stabilizing her respiratory status while safely introducing gentle mobility and functional activities. Through consistent, targeted therapy sessions, Loretta was gradually weaned from high-flow oxygen to nasal cannula support, reflecting steady improvement in her breathing.

Physical therapists worked closely with Loretta to initiate safe ambulation and build endurance, while occupational therapists focused on improving functional mobility, transfers, and self-care skills. These combined efforts helped reverse her physical decline, rebuild strength, and restore confidence. Loretta’s active participation in therapy also supported her emotional well-being throughout her recovery.

As her endurance and tolerance for activity improved, Loretta qualified for transfer to the acute rehabilitation unit to continue intensive therapy. In the ARU, she participated in up to three hours of combined physical and occupational therapy each day. Her therapy program emphasized strengthening, balance, coordination, and retraining for essential daily activities needed for independent living. Pain management strategies and energy-conservation techniques were also incorporated to support her chronic conditions and promote sustainable progress.

Through the coordinated efforts of the therapy teams at both the LTACH and ARU, Loretta made remarkable gains. She progressed from high-flow oxygen dependence to low-flow nasal cannula, regained the ability to ambulate safely with minimal assistance, and improved her independence with transfers and daily activities. Ultimately, Loretta achieved the strength and safety needed to return home, reuniting with her loved ones with renewed confidence and independence.

Loretta’s journey highlights the power of comprehensive, patient-centered rehabilitation and the dedication of the therapy teams who supported her every step of the way.

Ivan, 76, with a complex medical history that included coronary artery disease, heart failure, atrial fibrillation, diabetes, chronic kidney disease, and peripheral artery disease, faced a life-threatening event when he experienced hemodynamic collapse prior to a scheduled MitraClip procedure. He was admitted to HCA Memorial, where he required ECMO for circulatory and respiratory support, along with mechanical ventilation and continuous renal replacement therapy due to acute respiratory and renal failure. During his hospitalization, Ivan successfully underwent MitraClip placement to treat severe mitral regurgitation.

Despite this critical intervention, Ivan’s recovery was long and challenging. He was eventually transferred to a long-term acute care hospital with a tracheostomy, dependent on hemodialysis, and severely deconditioned.

At Kindred Hospital North Florida, Ivan’s progress began slowly. He was initially unresponsive and dependent on the ventilator. An interdisciplinary team, including nursing, respiratory therapy, and rehabilitation specialists, worked diligently to manage his complex medical needs while gradually weaning him from ventilatory support. Care focused on stabilizing his cardiovascular status, managing kidney function and diabetes, and introducing early mobilization to address his profound deconditioning.

As Ivan’s strength improved and his condition stabilized, he transitioned to an acute rehabilitation unit. More alert and engaged, he was able to participate in intensive therapy. Physical, occupational, and speech therapy teams worked with him daily to rebuild strength, improve mobility, and increase functional independence. His tracheostomy was successfully decannulated, and he progressed to managing activities of daily living with minimal assistance.

By the completion of his rehabilitation stay, Ivan had made remarkable gains. He regained the ability to walk, perform basic self-care tasks, and improve his overall quality of life. His journey—from ECMO support to LTACH stabilization and intensive rehabilitation highlights the power of resilience, perseverance, and coordinated interdisciplinary care that supported him every step of the way.
