Hello loves,

You may notice that you’re being asked to complete new consent and health history forms before your upcoming appointments. We’ve updated the wording on these forms to better clarify who you’ll be seeing during your visits.

It was recently brought to our attention that—even if your appointment is with Lindsay or Crystal—your confirmation emails and texts may have listed Kayla. Since I (Kayla) am a nurse and not an IT expert, the booking system likely wasn’t set up perfectly at the start. (Which is why I still prefer paper charting 🤣) We’re working through these little kinks over the next few weeks.

If you’re asked to complete consents and health history forms more often than the usual 6-month requirement, please bear with us. We truly appreciate your patience and understanding as we make these updates.

We can’t wait to continue serving all your needs to the fullest!
