Shouting out Ms. Donna Blaha for her amazing progress in our Rehab to Home program!

After undergoing an extensive hip fracture repair, Ms. Donna came to Aperion Care Greenfield needing almost full support with mobility and daily tasks. With a custom rehab plan and her own determination, she’s now walking with a rollator, joining in on building activities and even heading out on outings with her sister.

We’re so proud of how far she’s come and can’t wait to see her take that final step home! 💪

Congrats to Mr. David Osgood on successfully completing his short-term rehab stay at Aperion Care Greenfield! 🎉

With a rehab-to-home plan designed just for him, and a few focused days of physical, occupational and speech therapy, he made strong progress with the support of our nursing team.

Mr. Osgood is now discharged and continuing his recovery with home health care. We’re glad to have been part of his journey and wish him all the best as he settles back in!
