We are an employee owned, independent Home Hospice and Home Health company providing various levels of care to our clients throughout the community. We take pride in honoring individuals by respecting cultural diversity, delivering compassionate personalized care, and developing an all-inclusive plan of care tailored to each client’s needs. Our highly-trained team is available 24 hours a day, seven days a week. Services provided by our hospice team include, but are not limited to:
Home Health Aid: Services based on need, up to 7 days a week to assist with ADL’s
Nursing Team: RN, LPN, Wound Care Certified, Certified Dementia Practitioner
Volunteers: Hospice philosophy trained, social companion, home maker
Spiritual Care: Our hospice chaplains practice non-judgmental, spiritual care regardless of their background, religion or culture. We are sensitive to the many forms of worship and culture components of faith, and know that spiritual expression may not involve religion or religious practice. Social Worker: Expertise in working with ethnic, cultural, and economic diversity; family and support networks; multidimensional symptom management; bereavement; and navigating health care systems. Physician: Specialized in hospice and palliative medicine, works with doctors and healthcare professionals to align their treatments with personalized goals. Bereavement: Providing support to our client’s families for up to 12 months after loved ones’ passing
Continuing Education for Facility Staff: We provide in-service training for healthcare professionals interested in learning the clinical and emotional aspects of hospice

Personal Care Services, Private Duty Nursing, Supportive Care, Home-Making Services
