Greetings Greensboro Pediatricians Families,

We hope you are having a great year thus far. The new school year is right around the corner, and we recommend calling our office to schedule your child(ren)'s yearly Well Child Check (WCC). We now have increased availability to provide well child-care to our patients of all ages and have available visits for sickness and acute problems.

If you have any forms that need to be completed (ex. Sports/ Kindergarten/ Daycare/ etc.) for this upcoming school year, please check with our scheduling team to confirm that your child(ren) are up to date with their WCC and feel free to bring your forms in during the checkup visit (Please have the parent section filled out before your appointment).

Sports Physicals are different from Well Visits.

Health forms needed for sports, camps, school enrollment, etc., can be completed based on the annual well visit performed within the past 12 months.

Please note, any acute or ongoing medical problem discussed with the provider during a WCC visit, may incur additional office visit charges billed to your insurance. You may be required to pay a copay and/or charges may be applied to your out-of-pocket expense (i.e., deductible, co-insurance, etc.)

Friendly reminders:
Forms: Forms that are completed outside of checkup visits have a Form Fee per patient of $10 for daycare/school forms and $25 for FMLA forms. Please allow 2-5 business days for forms to be completed by our providers. Due to high demand, we are unable to accommodate urgent/time sensitive form requests. Valid state or government issued photo ID may be required at the time of pick up.

***Incoming Kindergarteners attending Guilford County Schools for the Academic Year 2025-26 must have a WCC ON or AFTER, August 25, 2024, for form completion. ***
