Reviewing imaging in person is a key part of our clinical practice. This is especially important as interventionalists, as a thorough understanding of the imaging is critical for diagnoses and targeted treatment. While radiology reports are helpful, in-person review of imaging can oftentimes pick up subtle findings that are critical in guiding treatment. Pictured here is a small annular tear—a tear in the outer part of the spinal discs--which can cause significant back and leg pain in patients. We find going over the imaging with our patients promotes excellent understanding of our choice of treatments!

The role of ketamine in anesthesiology and pain medicine is multifaceted! In anesthesiology, we routinely use ketamine as an anesthetic and as an adjunct in perioperative/acute pain control. Ketamine can be given as a single shot injection (bolus) and/or as an infusion. We also use ketamine infusions for chronic pain conditions including many neuropathic (nerve-related) pain conditions such as complex regional pain syndrome and trigeminal neuralgia. Ketamine infusions may also be used for cancer pain management, fibromyalgia, and migraine headaches. Individuals should be screened carefully to decide whether or not ketamine infusions can safely improve their chronic pain, and it is best administered by an anesthesiologist board certified in chronic pain management to assure safety of the infusion. The field of interventional pain management continues to grow, with exciting innovations and opportunities to provide sustainable benefits to our patients. In the past, much of our field was centered around temporary interventions. Newer technologies such as minimally invasive sacroiliac joint fusion for persistent sacroiliac pain are an example of the true breadth and expansion of interventional pain procedures to elicit structural change. At Aviator Pain & Spine, we strive to provide the full breadth of innovative interventional options to our patients. Traditionally, treatment for discogenic pain has been limited, and we are excited to have an additional tool to offer our patients. Sacroiliitis, or inflammation of the sacroiliac joint, is a significant but often underdiagnosed cause of back pain. The sacroiliac joints connect your lower spine to each hip bone. Sacroiliitis can be associated with back, groin, and even leg pain.

A combination of physical examination and diagnostic injection into the joint can confirm diagnosis. Management of sacroiliitis can include physical therapy, anti-inflammatory medications, joint injections with steroid, platelet-rich plasma (PRP), and radiofrequency ablation of the joint nerves.

Sacroiliac joint fusion can also be an effective option when the pain still persists. A minimally invasive option such as the LinQ device can help to stabilize the joint and reduce pain.

At Aviator Pain & Spine, Dr. Prabhakar treats vertebral compression fractures (VCF)—sometimes called spine fractures—which is when your vertebra, or the bones in your spine, break and collapse. A VCF can cause pain, impacting quality of life. Balloon kyphoplasty is a minimally invasive procedure to treat VCFs that stabilizes the fracture and provides pain relief.

In the balloon kyphoplasty procedure, balloons are inserted through small incisions on the back and are inflated to raise the collapsed vertebra and return it to its normal position. Once the vertebra is in the correct position, the balloons are deflated and removed. The resultant cavities are filled with bone cement forming an “internal cast” to support the surrounding bone and prevent further collapse. The procedure typically takes about one hour. Of the many procedures we do, balloon kyphoplasty is one of the most gratifying, with many patients reporting immediate pain relief and better functionality.

In a previous video below, we talked about Vertiflex device to treat lumbar spinal stenosis. There are other minimally invasive tools available as well, such as MILD (minimally invasive lumbar decompression) which is a great option as well. The first step is identifying whether you have lumbar spinal stenosis, doing a thorough history and physical, reviewing the imaging to figure out what is causing the lumbar spinal stenosis, and choosing the best intervention for your pain.
