HAVING THE TALK
It can be so difficult for family members to make end-of-life decisions without knowing what your loved ones wanted. Advance care planning allows you to make these plans before it becomes too difficult or impossible to make one’s wishes known.

Talk with your health care professionals and loved ones and document your wishes in legal documents and medical orders.

Some of the issues you may consider:
• Where you want to live out your final days
• What, if any, life-supporting measures you want to receive
• Who will be the person to carry out your wishes if you cannot communicate

We tend to avoid talking about end-of-life care so as not to upset loved ones but, in fact, having your plans in place and keeping them updated can help address loved ones’ greatest concerns about your physical comfort, emotional support, and spiritual preferences.
