Day Three!

Our Home Health Team is the best around! We offer world class Nursing, Health Aide, Homemaker, and Respite with clinicians that live in our community. We also offer a full suite of therapy services including PT/OT and ST. We partner with the Guthrie and Audubon County Hospital Therapy departments to provide therapy in your home with local providers!

We also partner with Elder bridge, Audubon and Cass County to provide grant funded and income based sliding scale fee services to our residents.

Our Clinical team specializes in meeting the needs of our community, from medication management to primary care and gap services we offer a wide range of services to meet your needs!

Our new provider Sara Van Effen specializes in women's health, mental health and chronic condition management. With a special interest in providing our community with wrap around services Sara works with our social work team to make sure that our residents have access to and are integrated with all the health services available to them in our county.

We have so many wonderful providers in Guthrie and Audubon County! We had the honor of getting to dedicate our Guthrie office to Dr Steven Bascom who has served the Guthrie area and on the board of health for over 40 years!
