EFCS-TCM Services are available to children,teens and adults who are experiencing significant emotional behaviors difficulties, mental and medical health needs. Individual case management services are designed to assist adults with a serious and persistent mental illness gain access to needed medical, social, educational, vocational and housing services within the community. Case managers meet wit
h clients at a variety of community locations; including residences that are convenient and easily accessible for those involved in the program. Who is Eligible
Adults age 18 or older
Adults who are diagnosed with a serious and persistent mental illness (SPMI)
Funding Prior to services, payment or insurance will be authorized. Client must be Medicaid eligible, a county resident, and/or self pay for services.

Assistance with helping the individual and/or the individual’s family access and obtain needed services

Children 17 years of age and younger (up to age 21 if the child is still in school)
Children who have a mental health diagnosis or symtomps behaviors and need to be evalutated by a qualified mental health professional
