We have two family practice doctors, one Physician Assistant, and four Family Nurse Practitioner seeing patients daily in Hamburg, Sidney and Tabor, Iowa. Dr. Woltemath brings over 28 years of experience in Family Medicine. She is board certified with the American Osteopathic Board of Family Physicians and also a member of the American Medical Association and American College of Osteopathic Family Practice. Dr. Woltemath, also a Hamburg resident, takes pride in serving her community and exhibits superb leadership skills. He received his Family Medicine degree from St. George's School of Medicine. Dr. Ourada also has a law degree. He is a member of the American Academy of Family Physicians and brings a strong sense of commitment to his patients. Jaylene Zanker is a Physician Assistant at the clinic. She received her doctorate degree in Biomedical Sciences before going on to become a PA. She has worked as an assistant researcher studying Alzheimer's disease, stroke, migraine, and Parkinson's disease; much of this data has been published in distinguished scientific journals. She is a member of the American Academy of PAs, as well as other scientific organizations. Brooke Hollars, DNP, APRN earned her Doctorate of Nursing in April 2024 and has nursing experience in medical-surgical and oncology care. Brooke leads the Wound Care Clinic at Grape Community Hospital, providing specialized treatment and advocacy for patients with complex wounds. She takes a patient-centered approach, focusing on individual needs. Outside of work, she enjoys hunting and fishing with her husband, and spending time with family and friends. Laura Isaacson, ARNP brings over 24 years of nursing experience to her role as a Family and Emergency Medicine Nurse Practitioner. Laura’s healthcare journey began in the U.S. Army, where she trained as a firefighter, first responder, and combat medic. She earned her LPN from Northwest Tech in Maryville, BSN from Clarkson College, MBA in Finance from Southeastern Oklahoma State University, and her MSN-FNP from Chamberlain College. Locally, she previously served at Grape Community Hospital and Shenandoah Medical Center as an RN and House Supervisor. Known for her practical, patient-centered care, Laura focuses on helping people set realistic goals for long-term wellness. Paulina Gossman, ARNP, is a Family Nurse Practitioner at Grape Community Hospital with a strong background in oncology and a passion for rural healthcare. She earned both her Bachelor of Science in Nursing and Master of Science in Nursing from Clarkson College in Omaha. Paulina has over eight years of experience as a Registered Nurse and has been practicing as a Nurse Practitioner for two years. After several years in oncology, she transitioned to family practice to help improve access to healthcare in rural communities. She believes in a team-based, empowering approach to patient care, working alongside her patients to help them make informed decisions for healthier lives. Shelbie Graham, DNP earned her Bachelor of Science in Nursing from Missouri Western State University in 2020 and completed her Doctor of Nursing Practice at the University of Arkansas in 2025. She began her healthcare journey as a care partner in a local nursing home and later worked as a registered nurse before becoming a Family Nurse Practitioner.
