Autumn View Health Care Facility in Hamburg, NY is seeking a compassionate and dedicated Registered Dietitian (RD) to join our team! This full-time, on-site role plays a vital part in supporting the nutritional needs of our residents and enhancing their quality of life.

🔹 Pay Range: $35–$37/hour

🛠️ What You’ll Do
▪️ Complete comprehensive nutritional assessments
▪️ Develop & implement individualized care plans

▪️ Provide nutrition education for residents & families
▪️ Ensure compliance with state, federal & facility guidelines

🎓 Requirements
✔️ Current Registered Dietitian (RD) credential (CDR)
✔️ Bachelor’s degree in Dietetics, Nutrition or related field
✔️ NYS Certification or eligibility (CDN)
✔️ LTC/SNF experience preferred
✔️ Strong communication & assessment skills
