Immediate Full Time/Part Time Home Health RN Case Manager.
Job Requirements:
Comprehensive knowledge and skill in current nursing practice.
Knowledge of the concept of team and primary care nursing and the implementation of both models of care delivery.
Knowledge of OASIS
Knowledge of the community resources.
Ability to make meaningful observations, evaluations and reports.
Ability to recognize the needs and concerns of patients, caregivers, and the multidisciplinary team to maintain therapeutic relationships.
Ability to provide skilled nursing care based on scientific principles, the nursing process, basic behavioral concepts, and intelligent professional judgment.
Valid driver’s license.
Minimum of 1 year Home Care Experience

The CNA/HHA position is a committed guaranteed (30 hours plus) schedule per week. We offer: Weekly Flat Rate Mileage, Quarterly Bonuses, 90 Day Reviews with Salary Increases!
Qualified Applicant Must: Have valid CNA license, reliable transportation, proof of auto insurance, valid driver's license, current physical and PPD within the year, Background check and fingerprinting (we will provide fingerprinting if needed).
