Hyaluronic Acid Dermal Fillers have taken quite a hit in the press lately. Headlines love to highlight the extremes; pillow face, overfilled, unnatural. But the truth about filler, when practiced responsibly and skillfully, is something entirely different.

When hyaluronic acid filler is legally sourced, medically approved, and thoughtfully placed, it can be one of the most elegant tools in aesthetic medicine. This past weekend I had the privilege of being in Irvine, California as part of Allergan’s Masters of Botox, an experience that was both energizing and incredibly collaborative.

Being invited into a room with a select few like minded, science and safety driven providers who are deeply committed to advancing aesthetic medicine is always inspiring. The conversations were thoughtful, the exchange of ideas was open, and the shared goal was clear: continue pushing our field forward while keeping science, safety, and patient outcomes at the center of everything we do.

A highlight of the weekend was learning more about TRENI, Allergan’s newest neuromodulator innovation that is expected to enter the market. It’s exciting to see the level of research and technology that continues to evolve within neuromodulators. Being able to explore this alongside colleagues who are equally passionate about anatomy, technique, and responsible innovation made the experience even more meaningful.

What I always appreciate about meetings like this is the reminder that aesthetics is never static. Our specialty grows through education, collaboration, and a willingness to keep learning. When experienced providers come together to challenge ideas, share techniques, and discuss patient safety, everyone benefits, especially our patients.

I’m grateful to Allergan for the invitation and honored to be included among this group of talented clinicians.

Weekends like this are a reminder that aesthetic medicine isn’t just about results, it’s about continual learning, responsible innovation, and raising the standard of care for our patients.

Not every line next to the eye is a Botox problem. And that’s an important distinction.

When we talk about crow’s feet, we’re usually referring to the dynamic lines created by the orbicularis oculi muscle when we smile or squint. Those lines respond beautifully to Botox because the treatment relaxes the muscle that’s creating the fold in the skin.

But here’s the nuance I want both students and patients to understand:
Sometimes what looks like “crow’s feet” is actually a structural change not just a muscle movement.
Over time we lose volume along the lateral zygomatic arch and within the preauricular space. When that support diminishes, the skin at the outer eye loses its scaffolding. The result can be creasing, shadowing, or etched lines that look like crow’s feet but don’t fully respond to neuromodulators alone.

In those cases, treating the muscle without restoring support may leave patients wondering why their “crow’s feet Botox” didn’t completely solve the problem.

This is where anatomy matters.
By restoring volume laterally, along the zygomatic arch and into the preauricular area, we can reestablish the structural support that smooths and softens those lateral eye lines in a much more natural way. Often, the best outcome comes from understanding when Botox is the answer, when volume is the answer, and when it’s a thoughtful combination of both.

For providers in training, this is a great reminder that not all lines are created equal.

For patients, if you’ve treated crow’s feet before but still notice persistent creasing, it may not be that the treatment “didn’t work”, it may simply be that the underlying issue is volume loss rather than muscle activity.

Good aesthetics isn’t about chasing lines.
It’s about understanding why they’re there in the first place.

Are your peptides helping your health… or quietly making it worse?

Lipopolysaccharide, or LPS, is a potent endotoxin (a toxic substance from certain bacteria) that can enter the bloodstream when products aren’t properly purified or handled. Even in very small amounts, LPS can activate receptors like TLR4 (the body’s “alarm system” for detecting threats) and trigger a cascade of proinflammatory cytokines (chemical signals that tell your body to create inflammation).

That response can lead to systemic inflammation (body-wide stress and irritation), affecting blood vessels, immune balance, and overall function. In more serious cases, it can contribute to endotoxemia (toxins circulating in the blood), with symptoms like fever, fatigue, and cardiovascular strain.

Nasolabial Folds

When I first started in aesthetic medicine, treatment was often focused primarily on the nasolabial folds. As our specialty has evolved, we’ve learned to look at the face more globally, understanding how each compartment contributes to overall balance and aging.

Today, when a patient’s primary concern is the nasolabial folds, we first evaluate the lateral aspects of the face near the hairline. These areas act as important lifting vectors. By strategically placing hyaluronic acid filler laterally, we can lift and reposition tissue, often softening the folds in a more natural way.

That said, the nasolabial folds are still an important area and, when indicated, should be treated directly, but thoughtfully. Careful assessment, conservative placement, and respect for facial anatomy allow us to soften the folds without creating heaviness or overfilling.

The goal is always natural, harmonious results that support the patient’s overall facial balance and keep them looking like the best version of themselves.
