Not quite ready to go home yet? need time to recover? We are here for you!

We recently launched “Your Pathway Home” a 30-day program at the Forum at Town Center specifically designed for seniors who are not quite ready to go directly back home from a hospital or rehabilitation center stay.
Thank you Sharon, our first resident who took advantage of this great offer. Such a lovely note you left us yesterday, safe and strong now to go back home.
