Very innocuous-seeming words can catalyze BIG problems far into the future! If you're wondering when it is your responsibility to offer your opinion on someone's weight or their food choices... the answer is never. πŸ˜‰

(‼️PS if you are a parent, you DO have some responsibility to select/prepare/present the food to your children... but even in that circumstance - they have ownership of what they choose to literally put in their mouth or not. If you are desiring to raise "eating competent" kids with a healthy relationship to their bodies, commentary on their choices is not required πŸ’š)

Be ready to navigate these back-to-school body stigma b***y traps! ‡️

1. Pre participation physicals... weight *may* be required, but there is no requirement for the student to see/hear/know it!

2. School clothes shopping... know that whatever size your child may be is OK - even if it is a larger size than last year. Refrain from making body comments and suggestions that certain items are better because they are "flattering" (aka make them look smaller)

3. Health and PE class can be dangerous traps for stigmatizing, anti-fat, and false weight based narratives about nutrition and fitness... pick your battles with the school, but know you are fully capable of untangling those thoughts with open conversation at home.

4. The snack police... don't become it! Let them eat! Yes, all of it. They know their needs and their hunger, and chances are they are utilizing WAY more energy than they did clothing around for the summer. You do not need to "manage" or police this!

5. Keep an eye on the aesthetic activity participants... all kids are vulnerable to weight-bias and body shaming - but if you work with or parents children in aesthetically focused or size-based activities (dance, ice skating, cheer, diving, wrestling, etc...) - be prepared to have very open and ongoing conversations about food and body. Non-judgmental communication can be a great step toward ED prevention!

1. 🩺There are very few clinical encounters for adults that require a precise, must-weigh-you-right-now level of information. Those are the exception rather than the rule. And yes, you have permission to decline the scale in most cases.

2. πŸ₯—Calories in, calories out is an antiquated and long since disproven description of weight regulation. There are over a hundred factors that influence your weight and food and exercise are only two of them.

3. ⛔️ Your desire to lose weight is a culturally constructed preference that is reinforced in the healthcare system. Not the other way around. And healthcare professionals who are advising "weight loss for health" are at a minimum, ignorant and poorly-informed and at worst, actively harming patients.

Remember that no matter the level of clinical expertise a professional brings to the encounter, YOU are the expert in your body and your lived experience!

There are LOTS of ways to set boundaries, lay out expectations, and facilitate productive conversation without being disrespectful or beligerant to the health care provider 🩺

🚩BEWARE 🚩 when you hear someone talking about what "has been shown"...

A lot (dare I say a majority??!?) of the mainstream conversation about health, weight, and nutrition is no better than a preschool game of telephone 😳

Do I expect everyone to understand research analysis methods? No.

Would you expect that public health policy and clinical standards of care are based in rigorous methodology? Yes, you would... and you would also be wrong. 🫣

BAD science does not facilitate GOOD policy, unfortunately.

I have been MIA as I'm now going on the 3rd week of COVID pneumonia... it took 2.5 years to reach me, and all I can say is I'm so grateful for the vaccine immunity I have which certainly prevented me from ending up in the hospital.

I'll be back in force soon, but the speed at which I do that is yet to be determined...πŸ˜‰

This cycle of weight stigma will not stop on its own. If you are a health pro 🩺...

I get it. You were trained this way.

Me too.

I was trained this way in medical school in the 90s... and I was trained this way in a graduate nutrition program as recently as 4 years ago... πŸŽ“

Very little (read: nothing) has changed to help health professionals understand the science of weight stigma and the effects of categorical discrimination on health outcomes.

It is challenging (nearly impossible) to recognize the bias that we continually reinforce in our practices when you are in the weeds every day seeing patients!

It is not your fault that you may not recognize these problems.

⏰ Good news: you've seen this. You've met me now. This is your wake up call ⏰

I will be the first to raise my hand and admit I was "that doctor" for 15 years πŸ‘‹

So when I am calling out health professionals for failing to practice the most evidence-based, ethical version of patient care...

What I'm calling out is:

❎ A social culture that makes is nearly impossible to separate our personal biases from our professional practice...

❎ A clinical environment that does not support (or reimburse for) talking, listening, understanding, taking our time...

❎ A bunch of normal people who grew up in the 80s, 90s, and 00s - and have had continuous life-long issues with body image, nutrition, food, and weight...

❎ An education that still contains teaching rooted in classism, racism, sexism, and has been grandfathered into the curriculum for no good reason despite all the valid evidence to the contrary...

❎ A panel of patients that come in asking for weight management and weight loss because they have been conditioned by the world to associate thinness with health...

So all this to say... I want to help.

If you are a health pro open to learning about these issue critical to your practice, critical to your patients, and critical to our professional future... ‡️
