We take appointments by phone from 8-11 & 1-4 M-Th. Friday only from 8-11:30. Most Major Medical Insurance accepted as well as Self Pay for those Uninsured. Uninsured patients new establishing $150 and established visits $75 per visit plus any labs, etc. Any expenses over the Office Visit will be billed to the patient. These patients must also pay by CC or check. All Copay's and account balances are due at each visit. You must also present your Newest Insurance Card. We request that you cancel within 24hrs. If you "No Show" for any appointment, you will be charged a $35 fee which isn't covered by your insurance. Due to the care we wish to provide our patients in need of an appointment, after 3 "No Shows" we will no longer be able to be able to see you.
