🎄✨ Our Angel Tree Campaign Has Begun! Transitions Healthcare Autumn Grove is excited to kick off one of our most meaningful holiday traditions — our Angel Tree Campaign! 💙🎁

Help us bring joy and a merry Christmas to our residents by picking an angel from our tree. 👼 Once you select an angel, please bring it to the Activities Department — they’ll match your number with a resident and share their holiday wish list. 📝

🎁 Please wrap all gifts and return them to the Activities Department by December 18th, 2025. Our team will make sure every gift is delivered to residents on Christmas Day. 🎅🎄

A heartfelt thank you to everyone who takes the time to think of our residents during this special season. Your kindness means more than you know. 💖✨

💨 🌬️ COPD Awareness Month reminds us how critical tailored respiratory and skilled nursing support is for older adults. 🩺 At Transitions Healthcare, our teams work closely with residents to monitor symptoms, support breathing comfort, and prevent complications — whether they’re with us for short-term rehabilitation or long-term care. 💗

🦓🚌 What a wild afternoon! Residents from Transitions Healthcare Autumn Grove took a scenic drive through Keystone Safari’s Drive-Thru Safari 🦒🐫—where they spotted more than 100 animals from 25 different species roaming free across beautiful open spaces! 🦌🐃

From zebras and yaks to buffalo and elk, everyone had a blast seeing these amazing creatures up close and enjoying the sunshine and fresh air. ☀️🌿

It was truly a day full of smiles, wonder, and unforgettable moments! 💚
