The MyPlaceCT website is the only comprehensive, one-stop resource that explains the many ways to make that happen. My Place CT is the state’s single source of independent, well-vetted information with no commercial agenda. You’ll find information, providers of services and
in-person help to live at home or in the community. My Place CT is also for families and caregivers looking for support for their loved ones. Care Through Community Help Network
We all want to live where and how we choose. But it isn’t always easy to find what you need to live independently at home. My Place CT supports a growing network of community partners to help you find the help you need. The goal is to have experts in every community who can navigate the system and help individuals find local services and support. Care Through Community partners can help with:
• Determining needs - Help assessing what you need
• Finding care and support – help finding and hiring programs and services
• Paying for care – accessing state benefits and ideas for affording help
• Housing and transportation – options and resources

Look for the purple How can we help? sign at senior centers, libraries and other locations in your community. If your town doesn’t have a network yet, you can call 211 for help finding what you need. Or, go to the Finding Care and Support section of MyPlaceCT.org. My Place CT is sponsored by the Connecticut Department of Social Services, in collaboration with 2-1-1/United Way of Connecticut and community partners statewide.
