1/25/2024. The day has officially arrived. Family Circles Healthcare is officially closed for practice. We are still processing things until the end of the month, but no more in office hours. Working as fast as we can on medical records, but please have patience, this is a big undertaking with only 1 person able to do it full time and a couple able to help out in between their jobs. It has been and honor and a gift to see your faces in our practice for the last 7 years and I wish we could go another 70+. We wish you all the best and will miss you!

I am starting the process of getting medical records on to CDs. Please send an email to recordsfch@gmail.com to update us on where they need to go, or if you would like to come pick them. We can schedule a time to meet you to make sure you get them. Due to the massive volume of records, unless they are under 25 pages, there is no way I can print them out on paper for everyone. It is a simple PDF file burned on to the discs, to make it easier for your next provider to view and download what they need. I am trying to include any medical records that have come from your providers before establishing with us, but if we did not ever get anything, I am unable to give you those back. As I get them done, we will reach out to those who have not indicated they have new providers to let you know they are ready to be picked up.

Medical records and how to get them...
For all patients that have already found a new P*P I will gladly get the records to that facility as quickly as I can. For those that are looking or just want to have their records with them - I am working on a process to get everybody's records downloaded to a CD or paper (only if it is under 25 pages). I will contact everyone as soon as I get them ready to arrange pick up, etc. This is not something that will happen overnight. There are over 2000 charts I will be sorting in the coming month or so.
If you have a chart- I will be in touch as noted above.

We realize that with the abrupt closing of our practice, that a lot of people have questions and concerns. At this time, we are being overwhelmed with calls and it is impossible to answer all of them and let everyone know what is happening, so here are the highlights:
1. Pam Lyon is leaving the practice on 1/11/2024.
2. We do not have a provider that can take over for her.
3. Everyone got a call stating that there was an urgent message on their portal, but very few people have actually been able to find it.
-Log on to your portal, click on My Health, click on Forms and Documents. At the bottom is a link for Urgent Provider Departure.
4. We are aware that practices are not taking new patients, or the wait time is months long, but we have no control over that.
5. At this time, Pam IS NOT seeing any patients except a select few. If your appointment has not been canceled, then you are on that list.
6. We will try to make sure all prescriptions are filled for at least 6 months (except controlled medications) if we are able. If you do need any refills, please be very specific as to what you will need and where it is to be sent. We will not be filling "all my meds".
7. PLEASE be kind to our staff, we have no control over any of this and are just trying to do the best we can.

Dear Family Circles Healthcare patients,
I hope this letter finds you in good health during these challenging times. It is with mixed emotions that
I share some news regarding changes within our healthcare team at Family Circles Healthcare. I regret to
inform you that Pamela Lyon, FNP, your dedicated healthcare provider, will be abruptly departing from
our practice and her last day will be January 11th, 2024.
Pamela Lyon has been an integral part of our team, and her abrupt departure, though unexpected, is
also met with sadness and gratitude for the care that she has provided to each of you. We understand
that your relationship with Pamela Lyon is important. We want to assure you that we are committed to
ensuring a smooth transition for your ongoing healthcare needs to an outside provider of your choice.
It is with the heaviest of hearts that I say this, but if we do not find a full-time provider before Pamela’s
departure, we will be forced to close down the practice. There will be more information to follow, if we
choose this path.
Here are the key details you need to be aware of:
1. Departure Date:
Pamela will be leaving our practice on January 11th, 2024. She will not be scheduling any new patient
visits from here on. Our focus will be on addressing the emergent needs of her current patient panel.
2. Continuity of Care:
We recognize the importance of continuity in your healthcare, and we are committed to making the
transition as seamless as possible. While we are actively looking for a new provider to replace her, but
due to a shortage of providers this will be very difficult to do in such a short time. We are asking our
current patients to transition to a new healthcare provider outside our practice. We will gladly facilitate
the transfer of your medical records to your chosen provider. Should you have any questions or concerns
about this transition, or if you need assistance in any way, please do not hesitate to contact us at 207-
270-4060 to initiate this process or at Recordsfch@gmail.com. We are here to support you during this
time of change.
3. Outside Providers:
Attached is a list of outside providers that are aware of our current situation and may be able to assist
you in your care. Please reach out to them as soon as possible.
We understand that change can be challenging, especially when it comes to your healthcare. We want to
express our gratitude for your understanding and assure you that we are here to help you during this
transition.

We will not be able to get phone calls, but please leave a voice-mail and we will try to monitor from there.

Recently, a nationwide shortage of various types of controlled medications, especially pain medication, has developed.

We understand this has the potential to be a very difficult period for many of you. However, we are doing everything we can to make this unfortunate situation as manageable as possible. We will try to assist as much as possible, but due to the volume of patients we care for, we can not always take time to call various pharmacies for you to see if they have any in stock. Please contact as many local pharmacies as you are able to to try to help. Keep in mind, not all pharmacies are willing to disclose if they have any in stock, so just please keep trying.

In the event you are not able to obtain your medications, please report to the nearest Emergency Department if you experience any symptoms of withdrawal, such as headache, body aches, tremor, seizures, diarrhea, nausea, vomiting, rapid heart rate, sweating, high blood pressure, fever, or any other concerning symptoms.
