CASE OF THE MONTH

History: A 41-year-old female was referred for blurry vision in the left eye for 1 week. She denied any viral prodrome, floaters, flashes, or curtain over vision.

Exam: Visual acuity was 20/20 OD and 20/80 OS. IOPs were normal OU and anterior segment exam was normal OU. Posterior segment exam and OCT OD were unremarkable. Posterior segment exam OS revealed a normal cup, peripapillary atrophy, and central RPE changes (Fig 1) and OCT was significant for irregularity the outer retina and RPE with ellipsoid zone disruption (Fig 2). Fluorescein angiography was normal OD and significant for staining of the RPE changes OS (Fig 3). OCT-A did not suggest presence of CNVM OS. Fundus autofluorescence demonstrated mild central hypo-autofluorescence. The patient was followed over 2 months during which her VA improved to 20/30, and OCT demonstrated ellipsoid zone reconstitution (Fig 4).

Differential Diagnosis:
-Acute Idiopathic Maculopathy
-Central Serous Chorioretinopathy
-Acute Posterior Multifocal Placoid Pigment Epitheliopathy
-Punctate Inner Choroidopathy
-Harada Syndrome

Discussion: Acute idiopathic maculopathy is a rare, self-limiting inflammatory disease of the macula that typically causes sudden, unilateral, and painless central vision loss in otherwise healthy young adults. VA can be reduced to 20/200 or worse at presentation and can be preceded by a flu-like illness which has been associated in some cases with coxsackievirus infection. Diagnosis is made clinically with patient history, exam, and imaging. Exam findings may include RPE changes, foveal/parafoveal thickening, serous macular detachment, and CME. OCT can be used to diagnose photoreceptor outer segment disruption and FA, FAF, and OCT-A can be used to assess for RPE abnormalities and presence of subretinal fluid or CNVM. Vision often recovers spontaneously over days to weeks, with most patients regaining near-normal acuity.

CASE OF THE MONTH

History: A 17-year-old patient presented with right eye (OD) blurry vision 1 month following a high-speed baseball striking his right orbit. His pain and initial hyphema had subsided but blurry vision remained.

Exam: Visual acuity was count fingers at 5 ft and 20/25 in the left eye (OS). Intraocular pressure was normal in both eyes (OU). Anterior segment exam was remarkable for mild pigment on the anterior capsule OD otherwise normal OU. Posterior segment OD revealed a 0.2 CDR with chronic inferior vitreous hemorrhage and a white linear streak through the fovea with mild epiretinal membrane. There was also a small subretinal hemorrhage inferior to the optic disc (Fig. 1 and 2). There were no retinal tears or detachments. Posterior segment OS was normal. OCT OD revealed a break in Bruch membrane with mild subretinal fluid and epiretinal membrane (Fig 3).

Differential Diagnosis:
-Myopic lacquer crack
-Angioid streak
-Choroidal neovascularization

Discussion: Choroidal rupture results from traumatic ocular compression disrupting the choriocapillaris and Bruch membrane. It frequently occurs concentric to the optic disc within the posterior pole. Initial injury may be accompanied by subretinal hemorrhage and sub-RPE hemorrhage in addition to hyphema, iris sphincter rupture, commotion retinae, vitreous hemorrhage, retinal tears, and orbital fractures given the degree of blunt impact.1 As the vitreous hemorrhage in this patient resolved, the characteristic white, crescent shaped lesion is seen. It is important to monitor for secondary choroidal neovascularization. Prognosis is dependent on the location of the rupture and any associated subretinal hemorrhage.

Illinois Retina Associates Announces Dr. Kourous Rezaei Receiving the Distinguished EVRS - European VitreoRetinal Society Award during the 2025 EVRS Annual Meeting in Mexico

Illinois Retina Associates is proud to recognize Dr. Kourous Rezaei’s distinguished participation as an invited speaker and awardee at the 2025 European Vitreoretinal Society Annual Meeting, held in Mexico.

As the EVRS Award recipient, Dr. Rezaei gave a lecture on Advancing Surgical Skills in the digital age and on an innovative educational platform designed to enhance surgical education for retina specialists around the world.

Dr. Rezaei had the honor of moderating a prestigious international surgical session in which leading retina surgeons from Turkey, Italy, Germany, Greece, Mexico, Croatia and Argentina were invited to discuss the lates surgical techniques in vitreoretinal surgery. Illinois Retina Associates is proud to recognize Dr. Kourous Rezaei’s distinguished participation as an invited speaker at the 2025 American Academy of Ophthalmology Annual Meeting, held in Orlando.

This premier global forum is the largest ophthalmology meeting in the world with over 10,000 health care professionals participating. It brings together leading retina specialists from across the world to share the latest advancements in the world of retina. Dr. Rezaei had the honor of moderating a prestigious international surgical session in which leading retina surgeons from the United States, Europe and Japan were invited to discuss the lates surgical techniques.
