Job Summary
Home Care Associates LLC is seeking a licensed Registered Nurse (RN) to conduct initial assessments, ongoing re-assessments, and supervisory visits for home-care clients. The RN ensures client care plans are accurate, compliant, and meet state and Medicaid guidelines.
Key Responsibilities
Conduct initial client assessments in the home
Perform 30-, 60-, and 90-day re-assessments as required
Develop, review, and update Plans of Care
Complete required Medicaid and agency documentation
Supervise and support personal care aides
Communicate changes in condition to families and agency leadership
Ensure compliance with state regulations and agency policies
Maintain accurate and timely clinical documentation
Qualifications
Active Registered Nurse (RN) license in the state of Virginia
Minimum 1 year nursing experience (home care preferred)
Knowledge of Medicaid home-care requirements (preferred)
Strong assessment and documentation skills
Reliable transportation and ability to travel to client homes
Professional, compassionate, and dependable
Compensation
