OUR SERVICES: Wisconsin Well Woman Program; Sexual Assault Nurse Examiner (SANE) Program; Podiatry Services; Diabetes Services; Home Visiting; Medication Management; Woman, Infant, Child (WIC); Birth-to-3 (B-3); HIV/AIDS Testing; Immunizations; Lead and Hemoglobin Testing; Optical Services; and Transportation Services.

OUR TEAM: Our Community Health Department have experienced registered nurses (RNs) and community health representatives (CHRs) whose primary focus is the health of our community.
