This week we’re celebrating National Nursing Home Administrator’s Week, and we want to recognize someone very special to our Facility- our Administrator, Beth Jackson.
Running one of the largest nursing facilities in Kentucky is no small task, This 200 bed facility is home to our residents.
Around here, being an administrator isn’t just about paperwork and meeting-it’s about being present for residents, staff and family every day.
If you happen to Beth this week be sure to wish her a Happy Administrator’s Week.

Today we celebrate the heart of our facility—our incredible employees. Every day you show compassion, patience, and dedication to the residents who call this place home. The care you provide goes far beyond tasks and responsibilities; you bring comfort, dignity, laughter, and kindness to the people who depend on us most.

This resident, John Gross was admitted in December 2025 requiring up to moderate assistance for activities of daily living (ADLs) and assistance with mobility and ambulation. Through participation in skilled therapy services, the John demonstrated significant improvement in strength, balance, and functional mobility. He has progressed to independence with all ADLs and ambulation, successfully returning to prior level of function (PLOF) and is now able to safely return home and live independently. Great job John! Come back and Visit us soon.

Who are we?

Hazard Health and Rehabilitation Center is a recently-renovated 200 bed care facility located in beautiful Perry County, Kentucky. Our Medicaid and Medicare certified facility features 24 hour nursing and physician services, as well as pharmacy, lab, and radiology services. Our services are catered to residents who may require either long-term care, or a short-term rehabilitation stay to regain health and mobility. Hazard Health and Rehabilitation Center takes pride in being able to offer high quality care, in a home like environment. We strive to provide our residents a high quality of life experience during their stay. Our facility welcomes visitors and our caring, professional staff will be glad to answer any questions regarding our services.
