For residents, the award honors individuals who have led extraordinary lives, achieved significant accomplishments, or made meaningful contributions to their community, either before or during their time in long-term care. We are very proud of Beverly and her inspiring story! We are blessed that she chose Four Oaks Healthcare Center as her care provider 💚

Nothing like a little friendly competition to kick off the weekend! Our residents Henderson Health & Rehabilitation Center brought the energy for a lively game of noodle ball — and the smiles say it all!

At ATS, we believe therapy should be effective and fun. Today’s activity wasn’t just a blast—it supported mobility, coordination, and social engagement. Team spirit was high, laughter was loud, and everyone was a winner!

We are so excited to announce that we have been recognized as a 2025 Silver Achievement in Quality Award recipient by the AHCA/NCAL. We are the only skilled nursing facility in Washington County and only 1 of 3 in East Tennessee to currently hold this honor! We are incredibly proud of our Four Oaks team 💚
