What Happens When You Have a “Shy Bladder” for a Drug Test
Some people—especially those with paruresis (shy bladder syndrome)—find it impossible to provide a urine sample under supervision. In these cases, DOT and non-DOT collectors follow a formal procedure:
The individual may be asked to drink up to 40 oz of water over a 3‑hour period to produce a sufficient sample.
If unable to provide 45 mL (or 60 mL for express testing), the collector reports it, and a medical review officer may require a physician's evaluation before labeling it a test refusal
