At Home Harmony is an in-home medical practice specializing in care for older adults with complex health needs. Our nurses continually monitor your data.

We get to know you and your family, and our medical team continually manages your care and helps with medications. Who we are:
Backed with our 20 years of experience, our fully trained and licensed professionals deliver the highest standard of personalized care. Our integrated care team consists of quality physicians, advanced practitioners, pharmacists, nurses, and care aides. We want you to rest assured your loved ones are in good hands, so we perform extensive background checks and hire the very best in the field. How it works:
• Our pharmacy solution provides specialized packaging, weekly home delivery, and recurring medication therapy management consultations with the client and their family.
• Effective care coordination is delivered to our patients by deploying our dedicated team of providers including Nurse Practitioners and Physician Assistants. We continuously monitor your chronic conditions with easy to use digital devices in your home. If we detect a change in your condition, we will reach out to you and your care aide as well as your doctor. Our care team will also proactively reach out to you on a recurring basis to ensure you are safe and comfortable at home. If there is a change in condition, our care team will engage to manage care coordination, communication, care referrals, and effective transitional care management if a hospitalization is required.
• Our home care aides visit our patients regularly to ensure the devices are functioning. This member of our team is an important communication conduit to the patient and their family. Our nurses monitor patient health biometric readings daily and coordinate with other providers regarding the patient’s care.

Introducing Jennifer, one of our talented and thoughtful care providers!

Jennifer began her nursing career in Progressive Care with Bon Secours. She became an NP 10 years ago and started out in retail health care with CVS MinuteClinic. She found her passion for the older population, so she began completing home assessments for Humana Insurance. Eventually, Jennifer oversaw COVID testing for CVS during the pandemic. After that, Jennifer worked for Landmark Health completing home visits for Medicare patients for 2 years.

When not working, Jennifer enjoys spending time with her adult children and four dogs. She is also an avid walker and swimmer!
