• Our pharmacy solution provides specialized packaging, weekly home delivery, and recurring medication therapy management consultations with the client and their family.
• Effective care coordination is delivered to our patients by deploying our dedicated team of providers including Nurse Practitioners and Physician Assistants. We continuously monitor your chronic conditions with easy to use digital devices in your home. If we detect a change in your condition, we will reach out to you and your care aide as well as your doctor. Our care team will also proactively reach out to you on a recurring basis to ensure you are safe and comfortable at home. If there is a change in condition, our care team will engage to manage care coordination, communication, care referrals, and effective transitional care management if a hospitalization is required.
• Our home care aides visit our patients regularly to ensure the devices are functioning. This member of our team is an important communication conduit to the patient and their family. Our nurses monitor patient health biometric readings daily and coordinate with other providers regarding the patient’s care.
